1. To apply for using the name of individual industrial and commercial households, according to the Regulations on Individual Industrial and Commercial Households and the Measures for the Administration of the Registration of Individual Industrial and Commercial Households, submit to the registration authority where the business premises are located:

1. Application for the Pre-approval of the Name of Individual Industrial and Commercial Households signed by the operator

2. A copy of the operator's ID card. Where an agent is entrusted, the Certificate of Entrusted Agent signed by the operator and a copy of the ID card of the entrusted agent shall also be submitted.

2. After applying for the name, according to the Regulations on Individual Industrial and Commercial Households, the Measures for the Administration of Registration of Individual Industrial and Commercial Households and the provisions of tax laws and regulations, submit the following materials to the registration authority where the business premises are located:

1. Application for Business Registration of Individual Industrial and Commercial Households signed by the operator

2. Copy of the operator's ID card

3. Certificate of use of the business premises

4. Business scope of application for registration.

if an agent is entrusted, the Certificate of Entrusted Agent signed by the operator and a copy of the ID card of the entrusted agent shall also be submitted.

According to Article 12 of the Measures for the Administration of Food Business License, to apply for a food business license, the following materials shall be submitted to the local food and drug supervision and administration department at or above the county level where the applicant is located:

(1) An application for a food business license;

(2) a copy of the business license or other qualification certification documents;

(3) documents such as the layout of major equipment and facilities, operation procedures and so on, which are suitable for food business;
