Notes on training and assessment of food safety management personnel in Shenzhen
(1) The following materials are required to register for training assessment: 1, registration form (with photos); 2. Copy of ID card (if the quota for free assessment is used, it must be signed by the unit to which it belongs, and marked "Agree with Comrade * * * to participate in the food safety administrator assessment"); 3. For advanced food production and catering services, provide a copy of high school education certificate or above; 4, the use of free assessment sites, must have a copy of the business license, license, organization code certificate, institution legal person certificate, private non-enterprise unit registration certificate or name approval notice (unit signature); 5. 1 one-inch color bareheaded photo (production certificate). The registration form can be downloaded from the food safety administrator training and assessment online or collected free of charge at the training point and the food license acceptance window of the market supervision bureau. If you register by telephone or online, you must submit the registration materials to the training point half an hour before the training assessment begins. Those who fail to provide materials as required shall not participate in the training assessment or the training assessment is invalid.
(II) Training and assessment fees The government subsidizes the training and assessment of food safety administrators, and each food production and operation entity in our city has a free assessment quota of 1 person. The training expenses are combined with government subsidies and shared by enterprises or individuals, and the training evaluation institutions charge appropriate training expenses according to the cost. In any of the following circumstances, you must participate in the assessment at your own expense. 1, arrange more than 1 to take part in the assessment, and pay for the part that exceeds 1; 2. Those who have already registered, but failed to participate in the assessment on schedule, need to re-register at their own expense; 3, the first assessment is unqualified, at their own expense to participate in the assessment again; 4. Individuals who are not appointed by the unit and voluntarily participate at their own expense.
(III) Attendance for food production, catering service and food circulation The training time for food safety administrators is tentatively set at 2.5 days, 2 days and 1.5 days respectively, and the specific time is arranged by the training point. Participants must bring their ID cards and registration materials to participate in the training and enter the venue in advance. They are not allowed to be late, absent from school or leave early. Attendance management will be implemented in the training. I have to punch in every class with my ID card. Those with incomplete attendance must make up lessons separately, otherwise they will not participate in the assessment.
(IV) Assessment In order to enable food safety administrators to master food safety-related knowledge in an all-round way and improve their comprehensive quality, the Measures for the Administration of Food Safety Administrators in Shenzhen (Trial) clearly stipulates that food safety administrators must receive unified training, but it is believed that some students have mastered relevant food safety knowledge through self-study or other means and achieved the training purpose. Therefore, if they meet the following conditions, they can register for the exam on site, online or by telephone: 1. Those who have participated in the training and have Man Qin records must choose the appropriate examination cycle according to their own time and study situation within one month after the training, and the examination point must be the same as the training point. 2, through self-study or other means to master the food safety related knowledge, you can choose the assessment agency to sign up for the exam according to my schedule and place. After successful registration, take the exam with your ID card. Those who fail to take the exam or fail to pass the exam after registration must re-register and take the make-up exam at their own expense.
(five) after passing the examination, the "Shenzhen Food Safety Administrator Training Examination Certificate" was issued. The certificate is valid for 3 years. Three months before the expiration of the validity period, you must participate in the training and assessment of replacement, and those who pass the certificate will be replaced. If you change your work unit, you must apply for a new valid certificate in time.
