Second, the materials needed to apply for a food business license
1, apply for the Food Business License and fill in the company information. In order to avoid filling in mistakes, you can go to the processing point of the Industrial and Commercial Bureau and fill in according to the staff's tips, paying attention to all the information must be true.
2. Business license and photocopy of the company.
3, food safety system management text, you can go to the copy shop to buy, or the designated stationery store. If there is no hanging safety system management text clause in the store, you must buy hanging, otherwise the on-site inspection of the industrial and commercial bureau may not pass.
4, food safety management personnel health certificate, training certificate copy. (Generally, it is a company legal person)
5. Copies of employee health certificates (at least 2 people), as long as the company has employees with health certificates.
6. Company plan and geographical location map.
7. Two copies of the legal person ID card.
8. A copy of the ID card of the entrusted agent.
Third, the way to apply for a business license
1. When going to the industrial and commercial department where the new address is located, you need to bring the same information as that provided by the new enterprise. You can get the Notice of Enterprise Registration File Transfer and the Notice of Relocation and Change Registration with complete information.
2. Hold the Notification Letter of Enterprise Registration File Transfer (the information is mailed by the industrial and commercial departments of the two places) to the original industrial and commercial registration department to go through the formalities of industrial and commercial data transfer, and the industrial and commercial department of the old address will issue the Notice of Approval for Enterprises to Move Out after accepting the Notification Letter of Enterprise Registration File Transfer.
3. After going through the formalities, the industrial and commercial department waiting for the new address calls the enterprise after receiving the industrial and commercial information of the enterprise, and goes to the new industrial and commercial department to handle the specific address change (fill in a change registration form and power of attorney) with the Notice of Relocation and Change Registration to the permit window seven days after completing the formalities.
