The principle of "one place, one license" is applied to the food business license, that is, a food business operator who has obtained the main qualification shall obtain a food business license when engaging in food business activities in the business premises. The same food business operator who engages in food business activities in different business premises shall obtain food business licenses respectively according to law. In the same business premises, a number of relatively independent business entities engaged in food business shall obtain food business licenses respectively according to law. According to the relevant laws and regulations of our country, the applicant shall be issued with the following administrative license certificate stamped by this administrative organ: 1. License, license or other license certificate; Two, qualification certificate, qualification certificate or other certificates; Three, the approval document or certificate of the administrative organ.

To apply for a new food business license, change the main format or business items of the food business license, rebuild or expand the food business premises, and apply for a food business license, the following materials shall be submitted to the food and drug supervision and administration department:

3. Rules and regulations for ensuring food safety, such as employee health management system and training management system, food safety administrator system, food safety self-inspection and reporting system, food business process and control system, food additive use publicity system, cleaning and disinfection of places and facilities, maintenance system, incoming inspection and inspection record system, and food storage management system. 1 copy;

4. 1 Copy of the identity certificate of the legal representative or person in charge, food safety administrator and handler, and the original inspection (the identity certificate of the legal representative or person in charge is not required when applying for on-site acceptance); 1 Copies of training and assessment certificates of food sales operators (except prepackaged foods sales), catering service operators (except small catering) and unit canteen food safety administrators, with the original inspection; 1 original power of attorney of the agent (fill in the corresponding column of the application);

Central kitchens, collective dining distribution units, and canteens with a dining population of 3,000 or more are required to obtain certificates of training and assessment for senior catering service food safety administrators, large restaurants and canteens with a dining population of 500 or more are required to obtain certificates of training and assessment for intermediate catering service food safety administrators, and others are required to obtain certificates of training and assessment for primary catering service food safety administrators. Require food sales units to obtain the certificate of training and examination for food safety administrators in the field of food circulation. Units engaged in food sales and food manufacturing and selling must obtain a certificate of training and examination for food safety administrators in catering services.

5. Letter of Commitment on the legal use of the business premises (fill in the corresponding column of the application). Commitment: this unit solemnly promises that it has obtained the declared address as the legal right to use its business premises, and the detailed address is true and correct; The declared business premises comply with the relevant provisions on environmental protection and fire control; If the legal use of the declared business premises belongs to residence, it has informed the relevant provisions of Article 77 of the Property Law of People's Republic of China (PRC), and obtained the opinions of interested owners, owners' committees, community residents' committees and community workstations, and agreed to change the house with the declared address into the business premises of food business units;

6. Where food is sold by vending equipment, the applicant shall also submit the product qualification certificate of the vending equipment, the specific location, the name, address, contact information of the operator, and the publicity method of the food business license;

7. If on-site inspection is required, submit 1 on-site acceptance inspection opinion;

8. To apply for the sale of cooked food and alcohol in bulk, a cooperation agreement (contract) with the linked production unit shall be submitted, and a copy of the food production license of the production unit shall be submitted;

9. Operators who provide homemade wine in catering services shall submit the finished product safety inspection report issued by a qualified third-party food safety agency (valid within 3 months from the date of issuance of the report).
