In order to further improve the prevention and control of epidemic situation in novel coronavirus, ensure enterprises to resume production and work, and facilitate people to travel, it is decided to implement the "health pass" system according to the principle of classification. The relevant matters are hereby notified as follows.

First, the conditions for handling the Health Pass. Workers and individual industrial and commercial households who live in Class II and III areas of our city and need to return to work may apply for a "health pass" if they have no symptoms such as fever, persistent dry cough and fatigue for more than 14 days, and the following circumstances are excluded.

Being diagnosed as a confirmed case, suspected case, positive infection in COVID-19, not cured or less than 14 days after discharge;

Close contacts of confirmed cases, suspected cases and positive infected persons in COVID-19 have not been released from medical observation;

Less than 14 days after returning from Hubei and other areas with high epidemic incidence;

There is a history of contact with people in high-incidence areas such as Hubei, and the isolation observation time is less than 14 days.

Second, the process. Voluntary declaration can be made in the following two forms.

Enterprises declare on their behalf. Encourage employers to declare the "Health Pass Card" on their behalf as needed, and one person shall fill out the "Tangshan Returned Persons Declaration Form". After the township government and sub-district office where the enterprise operates review the information integrity, the "Health Pass" will take effect with the official seal.

Individuals declare themselves. The applicant shall truthfully fill in the Declaration Form of Health Pass for Returnees from Tangshan City, and the Health Pass will come into effect after the information integrity is reviewed by the community neighborhood committee.

"Health Pass" and "Tangshan Returnees Declaration Form" are supervised by the Municipal Prevention and Control Office, uniformly printed by the prevention and control offices of counties and development zones on the website of "Tangshan, China" and distributed to towns and communities. "Health Pass" and "Tangshan Returned Personnel Declaration Form" can also be downloaded and printed by the applicant on the "Tangshan, China" website.

Third, the role of "health card"

Cardholders can be directly employed without isolation.

Cardholders shall not be restricted from going back and forth to work and residence.

Cardholders shall not be restricted from carrying out normal production and business activities in Class II and III areas of this Municipality.

Drivers of operating vehicles shall not be restricted from going up and down to expressway, returning to Tang Dynasty and Tang Dynasty, and driving on various expressway in Class II and III areas of this Municipality, and shall not be forced to take isolation measures. Special circumstances such as returning from Hubei Province shall be stipulated separately.

Cardholders in express delivery, logistics distribution, catering distribution and other industries are not restricted to carry out on-site services in the second and third categories of the city.

Cardholders are not restricted in renting houses and staying in hotels in the second and third categories of this city.

The Health Pass Card is valid within 14 days from the date of processing, and can be applied again after expiration.

Four. Related requirements

The second and third types of areas mentioned in this Notice are other municipal districts except Qian 'an City and Qianxi County, and shall be adjusted at any time according to the requirements of the Tangshan Leading Group for the Prevention and Control of Pneumonia in novel coronavirus.

"Health Pass" shall be voluntarily applied for by enterprises and individuals according to their needs, and departments at all levels shall not force it, and no unit or individual may charge any fees in any name.

The applicant shall take full responsibility for the authenticity of personal information and shall not conceal or resort to deceit. If the report is untrue and causes serious consequences, legal responsibility will be investigated according to law.

The employer should strictly check and know the basic situation of employees in detail. If false information of employees causes epidemic prevention and control accidents, the legal representative and responsible person of the enterprise will be seriously investigated. Enterprises should strictly implement various prevention and control systems in accordance with the requirements of epidemic prevention and control. Once the employees are found to have abnormal conditions, they should immediately take emergency prevention and control measures, stop using their Health Pass, and immediately report to the relevant epidemic prevention and control departments in accordance with relevant requirements.

"Health Link" cardholders should protect themselves and cooperate with temperature measurement when entering and leaving supermarkets, markets, communities, enterprises and institutions and other public places.

All localities and departments should strictly implement the provisions on the application, issuance and use of the Health Pass, widely publicize and take the initiative to serve, simplify the process, improve efficiency and prevent crowding.

All counties, districts and development zones should start to apply for the "Health Pass" on February 22nd 14: 00, and do it as soon as it comes. No delay or prevarication is allowed for any reason.

Discipline inspection and supervision departments at all levels and government supervision departments will supervise and inspect the implementation of the "health pass" system, and those who make things difficult for no reason, buck passing, inaction, slowness and chaos will be severely punished according to the law and regulations once they are reported and verified, and will be notified in the city.

Units and individuals responsible for handling the Health Pass shall keep the personal information provided by the applicant strictly confidential to prevent the disclosure of personal information.

After the COVID-19 epidemic was lifted, the "health pass" system automatically failed.
