Original Notice of Name Pre-approval (if the name is not used, it need not be submitted); 2, individual industrial and commercial households set up registration application (to the industrial and commercial registration authority to receive and fill in as required); 3. A copy of the applicant's ID card (both sides are affixed to the corresponding position of the application for registration of the establishment of individual industrial and commercial households); 4. Original ID card of the applicant (for viewing); 5. One recent one-inch half-length photo of the applicant (pasted in the corresponding position of the application for registration of establishment of individual industrial and commercial households); 6. If the applicant entrusts an agent, it shall submit the Certificate of Entrusting an Agent signed by the applicant and the identity certificate of the agent (the agent or agent shall show the original ID card for inspection); 7, family planning department certificate (some areas need to be submitted); 8. Proof of business premises: Submit relevant documents according to the following requirements according to different situations: (1) If the Property Ownership Certificate has been obtained, submit a copy of all the property ownership certificates signed by the property owner or stamped by the property right unit; (2) If the house used has not obtained the Property Ownership Certificate, a copy of the Land Use Certificate signed by the owner of the house or stamped by all units shall be submitted; (3) For newly-built houses on state-owned land, a copy of the Construction Permit issued by the construction department shall be provided if the House Ownership Certificate or Land Ownership Certificate has not been obtained; (4) If the house built on collective land is temporarily unable to provide the property right certificate, if the property right belongs to the township government, the township government may issue a certificate agreeing to use the site; If the property right belongs to farmers or village committees, the village committee where the house is located shall issue a certificate of consent to the use of the site; If a house built on its own land cannot submit the above certificate, the village committee or neighborhood committee where the house is located may issue a certificate of consent to the use of the site; (5) The market entry certificate issued by the market management department of the place where the supplier provides business. If the residence of individual industrial and commercial households is leased, in addition to the property right certificate of one of the above, it should also submit the use certificate of one of the following: if it is leased, it should provide a house lease agreement (the lessor should be the owner of the house and the lessee should be the individual industrial and commercial households applying for opening); In case of sublease, the sublease agreement (the lessor is the original lessee) and the certificate of consent to sublease issued by the owner of the house shall be submitted or stamped on the sublease agreement for confirmation; If it is provided free of charge, it shall submit a statement issued by the owner of the house to change the house into a business house. If it is an urban house, it shall also submit the Registration Schedule-Residence (Business Place) Registration Form and the certification documents issued by the local residents' committee (or owners' committee) that the interested owners agree to change the house into a business house. If it is a non-urban house, submit the relevant certificates stipulated by the local government. 9.
