(a) full-time and part-time food hygiene management personnel with health management system, organization and professional training;
(two) there are processing and business premises, cleaning, disinfection and other sanitary facilities and equipment that meet the sanitary conditions and requirements;
(3) Having the conditions and measures to control the pollution in the process of food procurement, storage and processing;
(4) The employees have passed the pre-job training and health examination;
(5) Other conditions stipulated by the provincial health administrative department.
The health permit handling process is as follows
1. The applicant fills in the corresponding application form and submits the application for food hygiene license in the acceptance window.
2. Submit the materials for handling the food hygiene license, the relevant certificates of the legal representative (person in charge, owner), the qualification certificate of the entrusted agent and other materials.
3. After acceptance, conduct data and on-site audit in accordance with relevant regulations, and make a decision on granting (or not granting) administrative license according to the audit results.
4. The staff handling the food hygiene license shall make a decision on whether to accept the application within five working days after receiving it, and make a decision on approval within twenty working days after accepting it.
5. Issue the food hygiene license within ten working days after making the approval decision.

Article 22 of the Detailed Rules for the Implementation of the Regulations on Hygiene Management in Public Places.
The state practices hygiene licensing management in public places except parks, stadiums and public transport. Operators of public places shall, after obtaining the business license issued by the administrative department for industry and commerce, apply to the health and family planning administrative department of the local people's government at or above the county level for a health license before starting business. The specific scope of health supervision in public places shall be announced by the health and family planning administrative departments of the people's governments of provinces, autonomous regions and municipalities directly under the Central Government.
Article 23 of the Detailed Rules for the Implementation of the Regulations on Hygiene Management in Public Places.
Operators of public places shall submit the following materials when applying for hygiene licenses:
(1) An application form for hygiene license;
(two) the identity certificate of the designated representative or person in charge;
(three) the schematic diagram, plan and layout of the address of public places;
(four) health inspection or evaluation report in public places;
(5) Health management system in public places;
(six) other materials required by the administrative departments of health and family planning of provinces, autonomous regions and municipalities directly under the central government. If the central air conditioning and ventilation system is used, the sanitary inspection or evaluation report of the central air conditioning and ventilation system shall also be provided.
