1. This application form can be downloaded from the website of the Municipal Food and Drug Administration.

2. This application should be printed or filled in with a pen or fountain pen.

3. The front page of the application materials should be the catalogue of application materials, and the attached materials should be sorted and bound in order, and stamped with the seal of the applicant page by page, or signed by the legal representative (or person in charge or owner) page by page; The attached materials shall be A4 paper, and the foreign materials shall be accompanied by Chinese translation; The contents in the materials shall be complete, clear and accurate, and the altered parts shall be stamped or signed.

4. Please read the relevant laws and regulations carefully before completing this application. The license categories in this application are in accordance with the provisions of relevant laws and regulations. 5. When providing this application, the applicant shall provide the following materials according to law: (1) a copy of the original Catering Service Permit or Food Hygiene Permit; (2) a copy of the business license; (3) the qualification certificate of the entrusted agent;

(4) Effective food safety training certification materials, such as main person in charge, food hygiene administrator and key position operators.

6. The applicant should carefully check the changes of the production and business premises, layout process and sanitary facilities approved by the original license, and truthfully fill in the Table on Changes of Production and Business Conditions of Catering Service Operators. If the production and operation conditions change, the following explanatory materials shall be provided:

(1) Table of changes in the area of food production and operation sites and main facilities;

(2) the schematic diagram of the changed food production and business premises and equipment layout, technological process and sanitary facilities;

(3) In case of any change in the legal use of food production and business premises, the corresponding certification materials for the legal use of food production and business premises shall be provided; (4) If environmental protection assessment should be carried out after the original hygiene license is issued, materials that meet the relevant provisions of environmental protection should be provided. 7. If there are changes to the application contents, such as the name of the enterprise, legal representative (person in charge, operator), road name or house number, license category and remarks, etc., it shall apply for license change according to law.

8. when the applicant receives the catering service license, he must provide the original of the original catering service license or the food hygiene license.
