(1) Application
The applicant fills in the application form and submits the following relevant materials at the acceptance window of the food and drug supervision department of each district and county:
1. Application Form for Food Circulation License (collected at the window of each district and county branch or downloaded online);
2. A copy of the Notice of Name Pre-approval;
the established enterprises and individual industrial and commercial households shall submit the original and photocopy of their business licenses, the newly established enterprises shall submit a copy of the Notice of Pre-approval of Enterprise Names, and the newly established individual industrial and commercial households shall submit a copy of the Notice of Pre-approval of Individual Industrial and Commercial Households (individual industrial and commercial households may be exempted from submitting it if they do not apply for the name of the shop name).
3. The use certificate of the business premises suitable for food business;
(1) Submit the original and photocopy of the property ownership certificate and the Plan of the Business Place for the own property;
(2) Submit the lease agreement, the original and photocopy of the lessor's property right certificate, and the Plan of Business Place;
(3) If the property right certificate is not obtained for the house, the following materials and the Plan of the Business Place shall be submitted respectively:
① If it is an urban house, the certificate of the real estate management department (completion acceptance certificate), a copy of the house purchase contract and the house sales license shall be submitted;
② if it is a non-urban house, relevant certificates stipulated by the local government shall be submitted;
③ if the rented house is a hotel or restaurant, submit a copy of the business license of the hotel or restaurant;
④ if military real estate is used as business premises, a copy of the military real estate lease license shall be submitted.
the applicant shall outline the "place for the business and storage of food circulation" with a red pen on the "Plan of Business Place".
(4) When filling in the business premises, the location should be specifically stated, and the house number and room number should be made clear. If there is no house number or room number, the reference should be clear.
4. Identification certificates of the person in charge, food safety management personnel and food safety professional and technical personnel;
(1) Enterprises and individual industrial and commercial households shall submit the original and photocopy of the identity certificate of the person in charge (such as ID card, passport of foreigners, etc.).

b. The letter of appointment of the person in charge of a partnership enterprise refers to the letter of appointment of all partners
c. The letter of appointment of the person in charge of a farmers' professional cooperative refers to the election resolution of the general meeting on the chairman, that is, the legal representative
③ If the food production, circulation or catering service license is revoked, the person in charge directly responsible for it.
(2) Original and photocopy of the identity certificates (such as ID cards, passports of foreigners, etc.) of food safety managers and food safety professionals.
① food safety management personnel refer to full-time or part-time food quality and safety leaders within the enterprise.
Professional and technical personnel of food safety refer to the responsible personnel engaged in food quality inspection or food safety inspection.
② The applicant must also submit the Employment Contract or Letter of Intent for food safety managers and food safety professionals.
if the person in charge is also a food safety manager or a food safety professional and technical personnel, it is not necessary to submit the Employment Contract or Letter of Intent.
③ If the license for food production, circulation or catering service is revoked, the person in charge directly responsible shall not engage in food business management within 5 years from the date when the punishment decision is made.
④ if a food business operator employs a person who is not allowed to engage in food production, operation and management to engage in management work, the original license issuing department shall revoke the license.
5. List of business equipment and tools suitable for food business;
6. Documents on the spatial layout and operation flow of business facilities suitable for food business;
Spatial Layout of Business Facilities (the purpose, area, location of equipment and facilities shall be indicated, and for bulk food, the layout shall reflect a separate sales area); Food operation process documents, such as supply and sales process, etc.
7. Text of food safety management system;
(1) The Food Safety Management System required by food business enterprises and food business operators whose licensed projects need on-site inspection includes: employee food safety knowledge training system, food incoming inspection system, food incoming and outgoing inspection record system, employee health inspection system and health file system, food delisting system, food storage and transportation system, cold chain sales system, etc., and shall comply with the provisions of relevant laws, regulations and rules. Operators engaged in food wholesale should also have a food wholesale sales record system.
(2) Food operators who do not need on-site inspection may combine the above systems and formulate them according to relevant requirements.
8. Other materials required by Shanghai Food and Drug Administration.
9. Supplementary explanation:
(1) If you entrust others to apply, you must also submit the original and photocopy of the power of attorney and the identity certificate of the entrusted agent or designated representative (such as ID card and passport of foreigners, etc.); If the entrusted agent or designated representative signs at the "signature of the applicant", the designated (entrusted) person shall specify in detail the "other entrusted matters and authority" of the designated power of attorney.
(2) About the license applicant
① If an established enterprise applies, the enterprise is the license applicant;
② If a newly established enterprise applies, the investor of the enterprise is the applicant for license;
③ if an enterprise applies for establishing a new branch, the enterprise that established the branch is the applicant for permission;
④ if an individual industrial and commercial household has been established or newly established, the owner shall be the applicant for permission;
⑤ circumstances of not being allowed as a license applicant:
a. The license applicant conceals the real situation or provides false materials to apply for a food circulation license, and the applicant may not apply for a food circulation license again within 1 years.
B. if the licensee obtains the food circulation license by cheating, bribery or other improper means, the applicant shall not apply for the food circulation license again within 3 years.
(3) The application materials and certificates submitted by the applicant shall be original. If a copy is required, it shall be signed (or stamped with the official seal) by the applicant or designated representative (entrusted agent) and marked "consistent with the original".
