2. The original business license and ID card of the legal representative (person in charge);
3. The original and photocopy of the health certificate of the relevant staff;
4. Business environment and layout planning of each functional area;
5. List of food production equipment and facilities and other related materials.
Process of handling food business license:
1. Submit materials to the Food and Drug Administration;
2. Relevant staff will pay attention to the business premises and check whether the items in the business premises are placed reasonably and whether the business area and dining area are separated.
3. If the production equipment, environment and sanitary conditions meet the requirements, a food business license shall be issued.
