According to laws, regulations, rules and relevant health standards and requirements, the contractor's office organizes sampling or on-site inspection, reviews and accepts the materials and the site, puts forward written review opinions, and reports them to the leaders in charge for review; The undertaking office shall, within 15 days from the date of accepting the application, hand over the examination opinions and application materials to the health permit acceptance office. 5. The Municipal Health Bureau will hand over the licensing decision and materials to the health licensing acceptance office within 2 days after making the health licensing decision; The hygiene license acceptance office shall notify the applicant of the license decision within 2 days, make a food hygiene license decision, and issue the food hygiene license to the applicant within 10 days from the date of making the decision. The catering industry and collective canteens also need to provide the following information: (1) Health and safety certification materials such as food additives, tableware detergents and disinfectants used; (2) Description of the layout or setting of sanitary facilities such as toilets, changing clothes, hand washing and disinfection, fly prevention, fly killing, dust prevention, rat prevention, insect prevention, tableware cleaning and disinfection, garbage disposal, and banquet samples; (3) Restaurants and restaurants with air-conditioning devices should provide health monitoring reports of their operating environment according to GB16153-1996 Hygienic Standards for Restaurants. 14.
