Application for food production license (in triplicate); 2 copies (3 copies) of the industrial and commercial business license, hygiene license and enterprise code certificate within the validity period; 3 copies of the ID card of the legal representative or person in charge of the enterprise (3 copies); 4 enterprise production site layout (3 copies); 5 copies (3 copies) of the production process flow chart of the enterprise with key equipment and parameters marked; 6 copies of enterprise quality management documents (1 copy); 7. Enterprises implementing enterprise standards must submit a copy of enterprise standards (1 copy); 8 enterprises that have obtained HACCP certification and export food hygiene registration (registration) certificate shall provide copies of the certificate (3 copies); (5) Small and medium-sized rice production and processing enterprises shall submit applications to the Municipal Bureau of Quality and Technical Supervision, and other food production and processing enterprises shall submit applications to the Provincial Bureau of Quality and Technical Supervision. Annual report and review of food production license (1) Apply in accordance with the Regulations of the State Council on the Administration of Production License for Industrial Products. (2) The annual report time is within the validity period of the certificate, and the certified enterprise shall submit the annual report on the necessary conditions for the enterprise to continuously ensure food quality and safety before the expiration of 1 year 1 month. (3) Submission 1. Enterprise annual self-inspection declaration form (in triplicate); 2. Original food production license; 3 copies of the business license and food hygiene license of the enterprise; 4 copies of enterprise quality management documents. (4) Accepting unit, Municipal Bureau of Quality and Technical Supervision. (V) Handling procedures: enterprises apply for accepting visas by self-inspection (written verification and spot check) (VI) Time limit for handling visas 1. Written verification, visa review should be completed within 5 working days after acceptance (excluding product inspection time). 2. For spot checks, visa review shall be completed within 7 working days from the date of acceptance (excluding product inspection time). Replacement and acceptance of food production licenses (1) Apply in accordance with the Regulations of the State Council on the Administration of Production Licenses for Industrial Products. (2) The enterprise applying for renewal shall apply for renewal 6 months before the expiration of the food production license. (3) Submission 1. Materials required to apply for food production license (see above); 2. Submit the original and copy of the food production license; 3. Submit a comprehensive evaluation report on the quality of processed food in recent three years (mainly including quality problems, quality accidents, supervision and spot checks, annual self-inspection reports, etc.). ).
