To put it simply, the procedure is-fill in the form, go to the department for physical examination, wait for the results (about 3 working days), make a health knowledge questionnaire, and go to the designated window to get the certificate. Remember to bring your ID card and two photos, one on the form and the other on the card. Prices vary from place to place, and those within 100 should be few!
