Building structure: It shall comply with Item (2) of Paragraph 1 of Article 8 of the Food Hygiene Law of the People's Republic of China; Article 14 of the Measures for the Administration of Food Hygiene in Catering Industry (Order No.11 of the Ministry of Health on October 6, 2111); Code for Design of Catering Buildings (JGJ64-89) Chapter 2, Chapter 3, Section 1, Section 2 and Chapter 4; Requirements specified in Article 6 of Hygienic Code for Catering Industry and Collective Dining Distribution Units. 3. Kitchen and catering room: it shall comply with Article 14 of the Measures for the Administration of Food Hygiene in Catering Industry; Code for Design of Catering Buildings, Chapter 3, Section 3; Requirements specified in Article 7 of Hygienic Code for Catering Industry and Collective Dining Distribution Units. 4. Food equipment: It shall comply with Articles 9, 12 and 13 of the Measures for the Administration of Food Hygiene in Catering Industry; Requirements specified in Article 8 of Hygienic Code for Catering Industry and Collective Dining Distribution Units. 5, tableware cleaning and disinfection facilities: should comply with the "food hygiene management measures" Article 25, Article 26; Requirements specified in Item (11) of Article 7 of Hygienic Code for Catering Industry and Collective Dining Distribution Units. 6. Health managers and employees: They shall comply with Articles 18 and 26 of the Food Hygiene Law of the People's Republic of China; Article 6 of the Measures for the Administration of Food Hygiene in Catering Industry; Item (1) and (4) of Article 14 of the Measures for the Administration of Food Hygiene License (No.498 [2115] of Health Supervision); Article 25, Article 26, Article 38 and Article 39 of the Hygiene Code for Catering Industry and Collective Dining Distribution Units. 2. Materials to be provided for applying for hygiene license for catering industry. 1. When applying for hygiene license for catering industry, the following materials should be submitted first: ① Application for hygiene review of construction project and application form for food hygiene license (1 originals each, with the official seal of the applicant); (2) Hygienic Evaluation Report on Site Selection and Design of Construction Projects (1 originals) issued by a professional technical institution that has obtained the qualification accreditation of the health administrative department according to law; (3) business license, enterprise name approval notice or institution legal person certificate (1 copies, original inspection); (4) Legal proof of business premises (such as house lease contract, real estate license, etc.) (1 copies, original inspection); ⑤ The general plan of the business premises and the layout plan of equipment and facilities (1 copies each); The food hygiene quality assurance system (including health management institutions, personnel training, health management system and internal health quality inspection system, etc.) (1 copies each); 7. The agent of the applicant shall submit the power of attorney of the enterprise (original) and the ID card of the agent (1 copies); 2. When applying for the completion and acceptance of the catering industry project, the following materials should also be submitted: ① Hygienic Evaluation Report on the Completion of Construction Project issued by a professional technical institution that has obtained the qualification accreditation of the health administrative department according to law (1 originals); (2) Registration Form of Basic Situation of Health Examination and Health Knowledge Training for Food Employees in Shenzhen (1 originals); ③ Health certificate and food hygiene knowledge training certificate of food practitioners in this unit (original inspection). Procedures 1. The licensing authority is Longgang District Health Bureau. Handling unit Longgang District Health Supervision Office and each street health supervision branch. 2. The application form can be collected free of charge at the acceptance window of Longgang District Health Supervision Office and each street health supervision branch. 3, within 11 working days after accepting the application, conduct hygiene review on the site selection and design of catering industry projects (including data and on-site review), and issue the approval letter of hygiene review or hygiene supervision opinions on the design of construction projects; After accepting the application for the completion of the catering industry project, the catering industry project will be completed and accepted within 21 working days, and the Hygiene Permit or the Decision of No Administrative License will be issued.
