First, publish a loss statement in the local newspaper (the statement should contain the number and disability level of the disability certificate)

Second, cut out the newspaper statement, newspaper name, time, etc. Together with my written application, the certificate from the unit where I work, the subdistrict office and the township (town) people's government, my ID card, a copy of the household registration book (the first page of the household registration book and the page with my name), a two-inch bareheaded color photo, three one-inch photos and all other materials, submit it to the civil affairs bureau of the county (city, district) where the household registration is located for review, and the county (city, district) civil affairs bureau will review it.
