According to the Food Safety Law and the relevant regulations of the State Administration for Industry and Commerce, starting from June 1 this year, the industrial and commercial departments will be responsible for issuing the Food Circulation Permit. Anyone who engages in food circulation business activities shall apply to the industrial and commercial department for a food circulation license, without applying to the health department for a food hygiene license. The guidelines for handling food circulation licenses are as follows:
I. Materials to be submitted for handling the food circulation license
(1) To apply for a food circulation license, the following materials shall be submitted:
1. Application for food circulation license;
2. A copy of the Notice of Name Pre-approval;
3. Relevant certificates of legal use of the business premises;
4. The identity certificate of the person in charge;
5. Power of attorney and the identity certificate of the entrusted agent or designated representative;
6 full-time or part-time food safety management personnel identification;
7. Food safety management system;
8. List of food business equipment suitable for its business activities;
9. Other materials required by the industrial and commercial departments.
(2) If an operator who already has the main qualification applies for adding food circulation items to the business project, it is required to submit the original and photocopy of the business license, and it is not necessary to submit the photocopy of the Notice of Pre-approval of Name. After verification on the spot, the original business license shall be returned.
Second, the handling requirements
(1) The applicant should sign and seal the application form. The newly established food circulation enterprise applies for a license, and the enterprise investor is the license applicant; An enterprise that already has the main qualification applies for a food business license, and the enterprise is the applicant; Individual industrial and commercial households apply for food circulation licenses, and I am the license applicant.
(2) The materials submitted for applying for a food circulation license shall be true, valid and conform to the legal form. The applicant shall be responsible for the authenticity of the submitted materials.
3. If a food circulation operator has obtained a food hygiene license before June 1 2009, the original license will remain valid within the validity period; if he voluntarily applies for renewal, if the items specified in the original license change or the validity period expires, the food circulation operator shall apply in accordance with the provisions of the Measures for the Administration of Food Circulation Licenses and obtain a food circulation license after examination by the examination and approval authorities.
