Application Materials

① Application for Food Circulation Permit;

② Name Pre-approval Notice or the qualification certificate of the operator (business license, institution legal person certificate, resident ID card, passport, Hong Kong resident ID card, Macao resident ID card and other valid identification certificates);

⑤ list of business equipment and tools suitable for food business;

⑥ the plan layout and surrounding environment map of the business premises suitable for food business (graphic or text description can be used);

⑦ text of food safety management system (food incoming inspection system, food incoming and outgoing records system, food regular inspection and cleaning system, food counter sales system, food return compensation system, unqualified food delisting system, employee health management system, employee education and training system, food safety accident emergency plan system, etc.);

⑧ Other materials specified by the Municipal Bureau.

Note:

A. If the operator is an individual industrial and commercial household, it is not necessary to submit the employment documents of the person in charge, food safety management personnel and technicians, food safety management organization and division of responsibilities when submitting the application materials.

B. if the applicant entrusts others to apply for permission, the entrusted agent shall submit the power of attorney and the identity certificate of the entrusted agent or designated representative.

A newly established food business enterprise applies for a food circulation license, and the investor of the enterprise is the license applicant; An enterprise that already has the main qualification applies for a food circulation license, and the enterprise is the license applicant; When an enterprise branch applies for a food flow < P > permit, the enterprise that established the branch is the permit applicant; If an individual applies for a new establishment or an individual industrial and commercial household applies for a food circulation permit, the owner shall be the applicant for the permit. The applicant shall sign and seal the application form and other materials, and be responsible for the legality, authenticity and validity of the materials submitted by him.
