Hygiene license refers to the hygiene license issued by units and individuals engaged in catering food production and business activities after examination and approval by the administrative department of health, and the license number is registered for the record.

There is no charge for the annual inspection of health permit. But there is a charge for testing items and other things in public places.

The following information shall be provided for the annual inspection of health license:

1. Application Form for Annual Examination of Health Permit;

2. Staff health examination and training certificate;

3, the original "health permit";

4, on-site supervision and acceptance records;

5. Product inspection report;

6, the Ministry of health, the health department approved products to provide a valid certificate of approval, approval, production license copy (food production enterprises);

7. Other materials required by the administrative department of health according to law.
