2 English reference Qingshu Yiqi decoction [Chinese medicine terminology Committee]. Terminology of Traditional Chinese Medicine (2004)]

There are about four prescriptions of the same name in Qingshu Yiqi decoction, among which the prescriptions recorded in Volume 4 of Wenjing Wei are commonly used. It consists of 5g of American ginseng, 3g of Dendrobium15g, 9g of Ophiopogon japonicus, 3g of Coptidis Rhizoma, 6g of bamboo leaves, 6g of lotus root15g, 6g of Anemarrhena asphodeloides, 3g of licorice, 0g of japonica rice15g and 30g of watermelon Cui Yi. It has the effects of clearing away heat and relieving summer heat, invigorating qi and nourishing yin, promoting fluid production and quenching thirst, and is a representative prescription commonly used in modern times to treat summer heat.

There are four prescriptions published in Wen Jing Wei, which are called Lectures on Prescription of Traditional Chinese Medicine. This prescription has a significant regulatory effect on nerve and endocrine function, water and electrolyte balance, and has a strong protective effect on important organs such as heart and brain. Therefore, this prescription has strong anti-hypoxia, anti-oxidation and anti-inflammatory effects, and should have a positive therapeutic effect on heatstroke. However, for severe heatstroke, especially those complicated with important organ failure, it is necessary to cooperate with western medicine's liquid therapy to correct the imbalance of water, electrolyte and acid-base, and at the same time cooperate with modern medical therapy such as physical cooling, reducing intracranial pressure and improving microcirculation, and we cannot rely solely on this prescription. Those who can drink water must drink plenty of water to prevent renal failure. In addition, lotus stem Chinese medicine believes that it can relieve summer heat, regulate qi and eliminate dampness, and japonica rice can replenish qi and strengthen the spleen, relieve anxiety and thirst, and stop diarrhea and dysentery, which have obvious therapeutic effects on heatstroke, but modern medicine has not fully studied its therapeutic mechanism. [ 1]

4.8 Modern indications: Qingshu Yiqi Decoction is suitable for heatstroke, summer heat, persistent, polydipsia and fatigue, which belongs to qi deficiency and body fluid deficiency [5].

4.8. 1 heatstroke often occurs in high temperature and high humidity environment, and it is a disease characterized by dysfunction of thermoregulation center, sweat gland failure and excessive loss of water and electrolyte. The common reason is that the ambient temperature is too high, and the human body can get heat; Increased heat production, such as heavy physical labor, fever, hyperthyroidism, etc. ; Heat dissipation obstacles, such as high humidity, obesity and poor air permeability of clothes; Sweat gland dysfunction. During strenuous exercise or physical labor in summer, excessive heat is generated and heat is obtained from the outside. If this heat cannot be dissipated by radiation, evaporation, convection and conduction, the body temperature will rise. When the body temperature is higher than 42℃, it will cause extensive damage to human cells and organ dysfunction. At this time, the central nervous system is first involved, and the cells of the brain and spinal cord are degenerated and necrotic, which will stimulate brain edema, cerebral hemorrhage, increased intracranial pressure and even coma. Secondly, high fever can cause myocardial ischemia and necrosis, promote arrhythmia and cardiac insufficiency, and reduce skin blood flow to affect heat dissipation; ARDS may be caused by pulmonary vascular endothelial injury. Acute renal failure can be secondary to dehydration and cardiac insufficiency; Excessive sweating leads to water loss and sodium loss, as well as hypoxia and acidosis; Insufficient blood perfusion in gastrointestinal tract may lead to stress ulcer, liver necrosis and cholestasis; Severe muscle injury, elevated serum creatine kinase, etc. There are four main clinical types of heatstroke: one is muscle spasm caused by severe sodium deficiency; Second, the loss of sodium in body fluids, obvious dehydration will lead to fatigue, nausea, headache, hypotension, orthostatic syncope and other signs of heat failure; Third, the heart rate increased to 180 beats/min, the pulse pressure increased, and the rhabdomyolysis; Fourth, acute renal and liver failure, DIC or abnormal behavior, seizures, and then coma, heart failure, pulmonary edema, brain edema and other manifestations. [5]

4.8.2 The etiological mechanism of summer heat is basically the same as that of heatstroke, except that fever is not as serious as heatstroke. [5]

4.9 pharmacological effects of Qingshu Yiqi decoction [6]

4.9. 1 Effect on endocrine system; The monarch drug Panax quinquefolium can make the anterior pituitary gland secrete ACTH, and increase the synthesis and release of adrenocortical hormone; At the same time, the total glycosides of Panax quinquefolium have anti-stress effect and improve the hypoxia tolerance of brain and myocardium. Panax quinquefolium has anti-fatigue effect, improves muscle strength, reduces the consumption of liver glycogen, reduces the level of blood urea nitrogen during fatigue, and reduces the high lactic acid content of the body after exercise. The diol glycoside contained in it has adrenocorticotropic hormone-like effect. American ginseng can also resist the inhibition of atropine on saliva secretion and produce body fluid. American ginseng also has antidiuretic effect. Ophiopogon japonicus can enhance the function of pituitary-adrenal cortex system and improve the adaptability of the body. Coptis chinensis not only has diuretic effect, but also has the function of secreting ACTH. Anemarrhena asphodeloides can inhibit the catabolism of adrenocortical hormone in liver, obviously increase the content of plasma corticosterone, and resist the feedback decrease of plasma corticosterone caused by dexamethasone. Anemarrhena sapogenin can inhibit Na+K+ATPase, reverse transcriptase and various polymerases. Anemarrhena sapogenin can alleviate the toxic and side effects of glucocorticoid. Anemarrhena polysaccharide can enhance adrenal cortex function and reduce ACTH secretion; Anemarrhena asphodeloides can reduce the serum T3 level of patients with hyperthyroidism and yin deficiency, and make T4 return to normal. Glycyrrhiza uralensis Fisch can stimulate hypothalamus-pituitary-adrenal cortex axis to produce adrenocorticoid-like effect and promote the synthesis of corticoids. Glycyrrhetinic acid is similar to corticosteroids in structure, which inhibits the inactivation of corticosteroids in the liver, thus indirectly increasing the concentration of corticosteroids; Glycyrrhizic acid has adrenocorticotropic hormone-like effect, while glycyrrhetinic acid has deoxycorticosterone-like effect; At the same time, licorice also has antidiuretic effect. It can be seen that the monarch, minister, assistant and envoy of this prescription have different degrees of promotion on hypothalamus-pituitary-adrenal cortex axis. The results of this effect are as follows: firstly, the increase of glucocorticoid secretion can regulate and promote the metabolism of the three substances, play a powerful anti-inflammatory and antipyretic role, effectively play a "licensing" role, and make other hormones play a normal regulatory role; Second, the increase of mineralocorticoid secretion makes Na+ and water remain and reduces the continuous loss, which is very beneficial to the serious loss of water and salt, high fever and tissue cell necrosis during heatstroke.

4.9.2 Effects on nervous system such as brain. Anemarrhena asphodeloides not only has antipyretic effect, but also can enhance the central inhibitory activity of rats by changing central transmitters or other factors, inhibit the sympathetic nerve center, slow down the heart rate, and inhibit the activity of adrenal DβH at the same time, so that the synthesis and release of CAS are reduced, which leads to the decline of the functional activity of sympathetic-adrenal system. FM 100 contained in Glycyrrhiza uralensis Fisch has certain anticonvulsant, analgesic and spasmolytic effects, and total flavonoids of Glycyrrhiza uralensis Fisch has protective effect on cerebral ischemia-reperfusion injury. Intravenous injection of glycyrrhizic acid can improve the activities of mitochondrial ATPase and lactate dehydrogenase in brain tissue after global cerebral ischemia-reperfusion and alleviate brain edema. This is very beneficial to treat heatstroke, such as excessive heat production, high fever, irritability, increased heart rate and brain dysfunction.

4.9.3 Effects on Cardiovascular System Monomer Saponin Rb&; Shy; 1, Rb2, Rb3 all have calcium channel blocking effects, and high-dose saponins can inhibit the excitability of sympathetic nervous system or block the increase of intracellular free Ca2+ concentration, so they have obvious antiarrhythmic effects. It has protective effect on myocardial cells, alveolar type ⅱ epithelial cells and platelet ultrastructure; It can reduce the excessive elevation of serum NE and 5HT, improve microcirculation, and alleviate acidosis and tissue hypoxia. It can activate superoxide dismutase and reduce the production of lipid peroxide. The decoction of Dendrobium candidum can weaken the vasoconstrictive effect of high concentration phenylephrine and 5HT. Dendrobium alcohol extract can reduce whole blood viscosity and plasma fibrinogen, inhibit ADP-induced platelet aggregation and prolong prothrombin time. Ophiopogon japonicus can significantly improve myocardial contractility and cardiac pumping function, increase coronary blood flow, and at the same time have anti-arrhythmia and anti-shock effects, which can quickly repair obviously damaged myocardial cells, reduce the damage of hypoxia to myocardial cells, and reduce the area of myocardial infarction and necrosis. Berberine contained in Rhizoma Coptidis can promote the influx of extracellular Ca2+ in myocardial cells, resulting in the increase of intracellular Ca2+ concentration. Small dose can excite the heart, increase its contractility and increase coronary blood flow, while large dose can inhibit the heart. At the same time, it has antihypertensive effect, and its effect is enhanced with the increase of dose. Its main mechanism is to directly dilate blood vessels, inhibit the motor center of blood vessels, antagonize adrenaline, inhibit various pressor reflexes and antagonize cholinesterase. Berberine can also reduce myocardial autonomy, prolong action potential duration and effective refractory period, eliminate reentry impulse, inhibit myocardial fast Na+ influx and possible Ca2+ channel block, thus playing an antiarrhythmic role. Berberine can reduce myocardial oxygen consumption, protect ischemic myocardium and significantly reduce the range of myocardial infarction. Berberine can significantly improve the hypoxia tolerance of heart, brain and whole body, and inhibit platelet aggregation. Anemarrhena asphodeloides has obvious inhibitory effect on platelet aggregation. Glycyrrhiza uralensis Fisch can reduce blood lipid and prevent atherosclerosis, and glycyrrhetin has antagonistic effect on arrhythmia caused by various reasons. Glycyrrhizic acid has the effect of angiotensin ⅱa 1 receptor agonist, which can narrow the scope of acute myocardial infarction, and licorice has the effects of inhibiting platelet aggregation and thrombosis. This has a positive therapeutic effect on heart dysfunction and circulatory disorder during heatstroke.

4.9.4 Effects on Digestive System and Metabolic Function The monarch drug Panax quinquefolium can promote the synthesis of protein in liver, brain, serum, bone marrow and other organs, especially the synthesis of protein in liver cells, and has the function of lowering blood sugar. Dendrobium candidum decoction can promote gastric juice secretion, help digestion, enhance metabolism, and reduce spontaneous intestinal activity and gastrointestinal propulsion. Maimendong decoction can reduce blood sugar and promote gastrointestinal tract, which increases with the increase of dosage, but it can inhibit gastrointestinal propulsion and tension contraction of small intestinal smooth muscle caused by neostigmine, acetylcholine and BaCl2. Berberine contained in Rhizoma Coptidis can reduce blood sugar by inhibiting the heterogenesis of hepatic glycogen and promoting the glycolysis of glucose in peripheral tissues. Berberine is beneficial to bile, promotes bile formation, promotes the excretion of conjugated bilirubin in bile, significantly inhibits experimental gastric ulcer and stress gastric ulcer, and significantly inhibits gastric acid secretion, which has strong anti-Helicobacter pylori effect and protective effect on gastric mucosal injury. Low concentration berberine can cause intestinal contraction, while high concentration berberine can inhibit intestinal contraction, which has a significant antagonistic effect on intestinal hypersecretion, diarrhea and death caused by Vibrio cholerae and Escherichia coli.
