Article 12: Where an individual operates a business, the operator shall be the applicant; where a family operates a business, a family member shall be the applicant who presides over the operator.

Where an agent is entrusted with the application for registration, change or deregistration, the applicant's power of attorney and the agent's proof of identity or qualification shall be submitted.

Article 13 To apply for registration of an individual industrial and commercial household, the applicant or the agent entrusted by the applicant may register directly with the registration authority in the place where the business premises are located; if the registration authority entrusts its subordinate industrial and commercial office to handle the registration of an individual industrial and commercial household, the registration shall be made with the industrial and commercial office in the place where the business premises are located.

The applicant or his authorized agent may submit the application to the registration authority where the place of business is located by mail, facsimile, electronic data interchange, e-mail and other means. By fax, electronic data interchange, e-mail and other ways to submit applications, should provide the applicant or his agent's contact information and mailing address. The registration authority to accept the application, the applicant shall be accepted within five days from the date of receipt of the notice of acceptance, submit and fax, electronic data interchange, e-mail content consistent with the original application materials.

Article 14 An application for registration of an individual industrial and commercial household shall be submitted with the following documents:

(1) the application for registration of an individual industrial and commercial household signed by the applicant;

(2) proof of the applicant's identity;

(3) proof of the place of business;

(4) other documents prescribed by the State Administration for Industry and Commerce for submission.
