1. Requirements for filling in the form

1. All the forms must be filled in quintuplicate with indelible ink pen, and any alteration is invalid.

2. It must be truthfully declared and filled in in a standardized way, and it is not allowed to conceal or falsely report.

3. All the contents to be filled in must be reviewed by the legal person and signed for approval.

4. Prepare all attachment materials and report them to the project approval department of the Environmental Protection Bureau. For all the copied materials, the Environmental Protection Bureau may ask for the original verification.

II. Attachment materials

1. Project name certificate

A copy of the Notice of Pre-approval of Enterprise Name submitted to the Administration for Industry and Commerce for newly-established enterprise or renaming;

for an enterprise that already has a Business License, submit its copy;

if it is a branch, submit a copy of the business license of the superior company.

2. Project Proposal: The Project Proposal must be submitted for the construction projects with key environmental protection management.

3. For construction projects that may have a significant impact on the environment, a preliminary environmental impact analysis report shall be submitted.

3. Guidance for filling in the form

1. The form in the upper corner of the cover is only for the environmental protection department to fill in.

2. "Investment in environmental protection" refers to the planned investment in the construction of pollution prevention and control facilities, such as waste gas, waste water and noise control facilities.

3. "Expected production date" indicates the expected production date after the completion of the construction project.

4. "Hotel area" indicates the total business area of the hotel.

5. "Restaurant area" is the business area of the restaurant. If there are more than one restaurant, please fill in it separately.

6. "Entertainment area" refers to the total business entertainment area such as ballroom, karaoke room and game room; Fill in the column of "DISCO ballroom" in the DISCO ballroom specially set up.

7. For "air conditioning equipment", fill in the model of air conditioner and indicate split air conditioner or central air conditioner.

8. "Fuel condition" If the fuel must be marked with heavy oil or diesel oil.

9. Fill in the total water consumption, wastewater discharge and repeated water consumption for "drainage situation", where the total water consumption is the sum of wastewater discharge and repeated water consumption.

11. "Pollution prevention measures of waste water, waste gas and noise" briefly describes the main pollution sources of the declared project and the pollution prevention measures to be taken (including construction period and operation period).
