To apply for a catering license at the window of the local food and drug administration, you need to fill out an application form.
Prepare relevant application materials. Generally, you only need to provide a copy of the name pre-approval notice, a copy of the ID card of the legal representative (person in charge or owner), a copy of the health certificate of catering service employees (the health certificate can only be processed after the physical examination of the local CDC), the house ownership certificate or lease agreement, the equipment layout of catering service units, the processing flow, the schematic diagram of sanitary facilities, the food safety management rules and regulations of large-scale catering, schools and kindergartens, the processing flow of key links, emergency plans and other materials. Some catering units with environmental risks around them also need to apply to the local environmental protection department for environmental impact assessment materials and water quality test reports issued by the CDC.
Restaurant food safety administrators should participate in safety training organized by food and drug departments and obtain training certificates.
Then submit the application form together with the above materials to the window of the food and drug supervision department. After the food and drug supervision department accepts it, it will generally conduct on-site audit and acceptance of the applicant's food and beverage outlets within 20 working days, and issue catering licenses after reaching the relevant national standards. Temporarily can not meet the standards, put forward rectification opinions, and issue a certificate after rectification.
