Procedures for applying for a Catering Service Permit:

Application-> trial-> on-site review-> issue a certificate

To apply for a catering service license, the following materials shall be submitted:

1, catering service license application;

2, a copy of the business license issued by the administrative department for Industry and commerce or enterprise name pre-approval materials;

3. A copy of the qualification certificate and identity certificate of the proposed legal representative or owner;

4, a copy of the site ownership or use right certification materials;

5. Plan of business premises, equipment, facilities, people flow and logistics with marked area;

6, catering service management organization and system, quality assurance system and quality control related documents;

7. Copies of Health Certificate and Health Training Certificate of the Owner and its employees;

8. Other information deemed necessary by the food and drug administration.
