Three. Procedures for handling food hygiene licenses
(a) the food production and business operation entity shall submit an application to the "window" of the Health Bureau of Maoming Investment Service Center with the above information, and fill in the "Application Form for Hygiene License". If the information is complete and meets the requirements, it shall be accepted; If the information is incomplete and does not meet the requirements, it will not be accepted, and the "window" will return the application materials, and then apply after completing the information.
(2) within three days after the health supervision institution accepts the health permit, it shall send two or more health supervisors to the site to conduct supervision and inspection, and put forward opinions on health supervision. The food production and business operation entity shall improve the sanitary facilities within a time limit and apply for the acceptance of sanitary projects.
(three) by more than two health supervisors to the scene for acceptance, after acceptance, health supervisors in the health permit application form signed health review opinions, reported to the Municipal Health Bureau for approval.
(four) the Municipal Health Bureau shall, within one day after receiving the application materials for the health permit, conduct an audit, and sign the examination and approval opinions if it meets the conditions for issuing the license; Those who do not meet the conditions for issuing certificates shall return the information.
(five) "window" license issuing personnel according to the examination and approval opinions signed by the Municipal Health Bureau, computer printing "health permit" and issued to the production and business units.
