3. Medical examination fees: according to the relevant charging standards of Minjia [1999] No.111 and Minwei [1999] No.99..

4. Physical examination procedure: I take the form, fill in the form, paste the photo, register the number, pay the fee, check according to the relevant items, and return the physical examination form.

5. Physical examination items: carried out in accordance with relevant specifications and the requirements of preventive health examination in the industry.

6. Other matters:

① The medical examiner must have two recent 1-inch bareheaded photos.

② The appointment physical examination form must be stamped with the official seal of the company.

③ Get a health certificate with valid certificate within one week after physical examination.

④ After the physical examination of occupational health (personnel with special types of work) is completed, the disease institution will inform its unit of the examination results in writing.

⑤ those who have objections to the physical examination results and need to be re-examined, shall be re-examined with a certificate issued by the unit or with a valid identity certificate.
