6. An application for health examination of the construction project. Fill in the column of "Project Category" as new construction, reconstruction, expansion or continuation, and fill in the blank with the word "None" (as above);
7, catering enterprises should have enterprise health management system (as above);

If the production and business premises of catering enterprises are located in underground space, the approval issued by the environmental protection department shall be provided; If the production and business premises of catering enterprises are located in residential buildings, it is necessary to provide proof that they will not disturb the people (according to Article 7 of the Regulations on Environmental Protection Management of Catering Industry, Commercial Service Industry and Cultural and Entertainment Industry, and Notice on Issues Concerning the Establishment of Enterprises in Residential Buildings [2112] No.119);

11. Hygienic Confirmation for Completion Acceptance of New Construction, Reconstruction and Expansion Projects (as above);
