How to reimburse maternity insurance

The maternity insurance reimbursement process refers to the procedure that the employer and employees themselves reimburse the maternity medical expenses, maternity allowance and other expenses of female employees during childbirth, and the nursing leave allowance of male employees during their spouses' childbirth to the overall regional maternity insurance fund.

The specific process is as follows:

The application materials that the employer needs to submit:

1. Social insurance registration form;

2. Increase and decrease table of personnel participating in basic pension, work injury and maternity insurance;

3. Summary of basic pension, work injury and maternity insurance declaration for enterprise employees.

The application materials that female workers need to submit:

1. Family planning certificate (original and photocopy) issued by the family planning department;

2. Birth (death) certificate issued by the medical department (original and photocopy);

3. My ID card (original and photocopy) of the maternity female worker and the family planning operation worker;

4. Examination form of maternity medical certificate for enterprise employees;

5. Application form for medical certificate of family planning operation for enterprise employees;

6. Application form for reimbursement of maternity medical expenses of enterprise employees;

7. Approval statement of maternity insurance benefits for enterprise employees;

8. Application form for maternity insurance for enterprise employees in other places for medical treatment;

9. Original materials such as bills of maternity medical expenses, expense lists, outpatient medical records and discharge summaries;

11. Receipt.
