The project undertaker shall fill in the sample registration summary table.

Six, data spreadsheets should be made by Excel software, and electronic documents should be written by word software. Paper materials need to be signed by the summary person and the reviewer, and stamped with the official seal of the unit. The summary table of sampling results also needs the signature of the approver. The paper materials submitted must be consistent with the data spreadsheet.
