2. Enterprises apply to the health and epidemic prevention department for a health permit.

Hygiene license is a certificate of hygiene license issued by units and individuals engaged in food production and business activities of restaurants after examination and approval by the administrative department of health, with the license number registered for the record.

First, the order of applying for the permit:

Apply for the drawings (and prepare the required materials at the same time). Apply for the materials after the drawings and related materials pass the on-site acceptance

Get the hygiene license within 11 working days

Second, apply for the hygiene license:

1. A floor plan of the production and business operation site, with the scale (1: 111) and indoor functional rooms indicated.

2. Certificate of use of the production and business premises (certificate of property rights and lease agreement);

3. a certificate of qualification of the legal representative or person in charge (resolution of the board of directors, articles of association or appointment document);

4. a schematic diagram of the address and orientation of the production and business operation place: the direction, street name, the relationship between the location of the production and business operation place and the surrounding buildings and adjacent units shall be indicated;

5. Training certificate of legal person or person in charge.
