Diet after total gastrectomy
Principles of rehabilitation and diet adjustment after gastrectomy —— First, get out of bed as soon as possible after gastrectomy. Due to the trauma and anesthetic effect of abdominal surgery on the intestine, patients usually have symptoms of intestinal paralysis such as abdominal distension and no exhaust after operation. In order to promote the recovery of intestinal peristalsis and avoid adhesion between intestine and surgical incision, doctors generally require patients to get out of bed as soon as possible. At the same time, long-term bed rest will lead to slow blood flow, which is easy to form thrombus for middle-aged and elderly people with arteriosclerosis and diabetes. Early to bed and early to rise can prevent lower extremity thrombosis and pulmonary embolism. Get out of bed as soon as possible, that is, 1 ~ 3 days after operation. According to the patient's physical condition, the sooner the better. Elderly or infirm patients can move in bed or indoors with the help of nurses or their families. Getting up early is easier said than done. Some patients are afraid of pain after appendectomy, and dare not move in bed or even turn over, which leads to intestinal adhesion before discharge and intestinal obstruction. Some people have to have a second operation. Second, wait for the exhaust before eating. If the patient can follow the advice of medical staff, get out of bed early after operation, recover intestinal peristalsis faster, and generally exhaust (fart) faster. Exhaustion shows that the patient's intestinal peristalsis has basically recovered, and he can eat a small amount of liquid diet, including noodle soup, rice porridge and pork floss. The intake of liquid diet is a good stimulus to the intestine, which can promote the recovery of intestinal function and is conducive to rehabilitation. Third, diet conditioning should be gradual. Subtotal gastrectomy is a typical gastrointestinal operation. Generally, you can drink some water 3 ~ 5 days after operation, that is, boiled water, fruit juice and soybean milk without residue. Every 2 ~ 4 hours, 30 ~ 50 ml each time. In a couple of days, I will eat all liquid foods, including rice porridge, millet porridge, pork floss, milk and so on. , 5 ~ 6 times a day, 80 ~ 100 ml each time. In two or three days, you can switch to a semi-liquid diet, which is rotten noodles and noodles. After 12 days, the patient's gastrointestinal function basically recovered and he could eat normally. After subtotal gastrectomy, the size of the stomach becomes smaller. During the recovery period, we should eat gradually, eat less and eat more meals to avoid eating too much at one time, which will cause gastric dilatation and anastomotic tear. At the same time, we should chew slowly and try not to eat sticky, cold, raw and skinned food, so as not to cause infarction through anastomosis. Gastric tumor, gastric ulcer bleeding and pyloric obstruction are often treated by gastrectomy. After the operation, the gastric cavity becomes smaller and the structure of the stomach changes, which affects the normal physiological function of the stomach, often leading to poor appetite and fullness after meals, leading to digestive and absorption dysfunction. So it is more important to adjust your diet at this time. After gastrectomy, patients usually recover their intestinal function within 1 ~ 3 days. When intestinal gas is discharged from the anus, they can eat a small amount of clean food, such as rice juice, thin lotus root starch, honey water, noodle soup, vegetable soup, etc., and drink100 ~150ml every time, 6 ~ 7 times a day. After 3 ~ 5 days, it should be changed to liquid food, such as rice porridge, millet porridge, egg soup, steamed egg soup, egg batter and so on. And eat 5 ~ 6 times a day. One week after operation, you can eat semi-liquid diet, such as noodles, wonton, millet and jujube porridge, steamed bread, bread, soda biscuits, stewed tofu, steamed fish, stewed fresh and tender vegetable powder and so on. Gas-producing foods such as milk and soybean milk and foods containing a lot of crude fiber such as celery, soybean sprouts and onions are not suitable for eating. Generally speaking, we should follow the following dietary principles: 1, insist on eating less and more meals, eat less every meal, 4 ~ 5 meals a day to adapt to the characteristics of small stomach capacity, and never overeat. 2. Prevent anemia. After gastrectomy, due to the decrease of gastric acid, the peristalsis of the upper part of small intestine is accelerated, which disturbs the digestive physiological function, thus affecting the absorption of protein and iron, and then iron deficiency anemia is prone to occur. Therefore, patients can eat lean meat, fish and shrimp, animal blood, animal liver and kidney, egg yolk, bean products, jujube, green leafy vegetables, sesame paste and other foods rich in protein and iron. 3. Because the physiological function of the stomach is weakened, do not eat cold and hard food at ordinary times, and avoid eating spicy and irritating condiments, such as pepper and mustard. It is forbidden to drink strong wine glasses. The following porridge for nourishing stomach, nourishing blood and promoting digestion is recommended. (1) 50 grams of stomach-nourishing porridge stem rice, 20 grams of lotus seeds, and 0/0 grams of jujube/kloc, with appropriate amount of water, are cooked with slow fire and eaten in the morning and evening, which has the effect of nourishing the stomach and strengthening the spleen. ⑵ 25g of Coicis Semen, 25g of glutinous rice, 0/0g of dried auricularia auricula/Kloc-0 (soaked) and 50g of pork liver (chopped) are used for nourishing blood porridge, which is beneficial to the treatment of iron deficiency anemia. (3) 50 grams of japonica rice, 25 # minced lean meat, diced white radish 100 grams, and 5-8 slices of hawthorn are used for digestion porridge, which can promote gastric acid secretion and help digestion.
