The dosage, concentration and speed of potassium supplementation depend on the clinical condition, blood potassium concentration and the improvement of potassium deficiency pattern in ECG. The concentration of potassium is not more than 3.4g/L (45mol/L), the rate of potassium supplement is not more than 0.75g/ h (10mmol/ h), and the daily amount of potassium supplement is 3-4.5g (40-60mmol). When potassium deficiency in the body causes severe rapid ventricular ectopic arrhythmia, such as torsade de pointes, short bursts, repeated multi-line ventricular tachycardia, ventricular flutter and other life-threatening serious arrhythmia, the concentration of potassium salt should be high (0.5%, even 10g%) and the dropping speed should be fast,1.5g/h (20mmol). If the condition is critical, the concentration and speed of potassium supplementation can exceed the above provisions. However, it is necessary to observe blood potassium and electrocardiogram closely and dynamically to prevent hyperkalemia. The daily dose for children is 0.22g/kg (3mmol/kg) of body weight or 3g/m2 of body surface area.
For example, potassium chloride preparation is 10% solution during potassium supplementation, so it cannot be directly injected intravenously to avoid myocardial inhibition and death. Generally, the concentration of intravenous injection is 0.2%, and the maximum is not more than 0.3%. That is to say, the solution of 1000ml cannot exceed 3ml of potassium chloride at most. 250ml is 0.75ml potassium chloride. Then 10% potassium chloride is 7.5ml.
The speed should not exceed 60 drops/minute, but patients with severe hypokalemia should be treated specifically. It is generally believed that the concentration and speed of rehydration are not the main problems, but the key lies in the amount of infusion per unit time. It is also safe to slow down the dropping speed when the concentration is high, and the method should be mastered flexibly.
The formula of potassium supplement (expected value-measured value) is multiplied by body weight and then multiplied by 0.3/ 1.34 to obtain 10% ml of potassium chloride. Usually use half first.
