1, general treatment: diet control, prevent obesity, avoid eating high-purine foods, such as animal viscera, beer, seafood and beans, etc., and eat low-purine, low-fat, low-salt and low-protein diets, and should abstain from alcohol and eat more alkaline foods to prevent acute gout and facilitate uric acid excretion. The daily drinking amount should be more than 2000ml.
2, acute attack treatment:
The inducement should be removed and the acute attack of arthritis should be controlled. The commonly used drugs are:
(1), colchicine:
0.2- 1.0mg every 2 hours 1 time, until nausea and diarrhea occur, the total amount should not exceed 6mg in 24 hours, but attention should be paid to monitoring adverse reactions, and the addition of non-steroidal anti-inflammatory drugs can reduce the dose of colchicine.
(2) Non-steroidal anti-inflammatory drugs: Stop taking drugs after symptoms such as indomethacin, naproxen, voltarin and celecoxib are controlled, and pay attention to monitoring the serum creatinine level during medication.
3, intermittent and chronic arthritis medication: The main purpose is to reduce the level of serum uric acid and prevent another acute attack.
(1), uric acid production inhibitor: allopurinol 0. 1g three times a day, which can be reduced to once a day according to the blood uric acid level. Some patients may have side effects such as fever, rash, abdominal pain, diarrhea, leukopenia and thrombocytopenia.
(2) Drugs for promoting uric acid excretion: benbromarone 50mg daily 1 time to 100mg daily 1 time; Probenecid 0.25g twice a day gradually increased to 0.5g three times a day. (It should be noted that uric acid lowering drugs may induce acute arthritis, so they should not be used in the acute phase).
4. Does asymptomatic hyperuricemia need treatment: mainly to lose weight and remove the inducement, such as the application of low-dose aspirin and low-purine diet. When uric acid increases, uric acid lowering drugs can be applied, and at the same time, combined hypertension, hyperlipidemia and hyperglycemia can be actively treated.
1, adjustment
(1), acute and early use of drugs, timely control of inflammation, rapid termination of acute attacks.
(2) When the blood uric acid increases to a certain extent, drugs that excrete uric acid or inhibit the synthesis of uric acid should be used to restore the blood uric acid concentration to normal, so as to prevent the occurrence of this disease.
(3) Prevention and treatment of complications caused by the deposition of sodium urate crystals in joints, kidneys or other parts. Prevent uric acid stones from forming.
(4) Prevent or treat diseases that can worsen gout, such as hyperlipidemia, hypertension and obesity.
