Off-site treatment

Eczema has better curative effect besides oral medicine (food) and external treatment.

External treatment: (1) Radix Arnebiae oil: 9g of Radix Arnebiae oil, fried with fragrant oil until the oil turns purple, and then the oil is taken out and applied to the affected area several times a day.

2, egg distillate oil: 7 eggs, cooked to take the yolk, put 50 ~100g sesame oil in the pot, simmer the oil in the yolk until the yolk is burnt, take the oil and apply it to the affected area frequently.

3. cuttlefish bone grinds into a proper amount of fine powder and spreads it on eczema several times a day, which is suitable for those with exudation.

4. Meretrix meretrix powder: Meretrix meretrix100g, pepper 50g, calomel 3g. Beat Meretrix meretrix into small pieces, stir-fry until golden brown, stir-fry with Sichuan pepper until black, and put it in a sealed jar when it is first smoked. Guarantee paste: 60g rosin powder and 30g sulfur powder are evenly ground, and sesame oil is mixed into paste, spread on gauze, rolled into strips as thick as fingers, tied tightly with thread, soaked in sesame oil 1 day, and taken out. Light one end with fire and then use a thick bowl. Cut off the cloth dust one after another, take the dripping medicinal oil, soak it in cold water overnight, and apply it externally, especially to patients with local infection. (6) Wash potatoes, chop them up and mash them, apply them to the affected area, wrap them with gauze, and change the medicine 2-3 times a day in the morning and evening.

Second, western medicine.

1, general therapy:

① Try to find the cause, isolate allergens and avoid re-stimulation. Excise the focus and treat chronic diseases of the whole body, such as dyspepsia, intestinal parasitic diseases, diabetes, mental nerve abnormalities, varicose veins of the lower leg, etc.

Pay attention to skin hygiene, do not wash skin lesions with hot water or soap, and do not use irritating antipruritic drugs.

(3) No alcohol, spicy food, fish and shrimp and other foods that are easily sensitized and difficult to digest, and pay attention to the relationship between diet and illness.

④ Work and rest should be combined to avoid excessive fatigue and mental stress.

2, systematic therapy:

① antihistamines: H 1 receptor antagonists have the functions of calming, relieving itching and reducing exudation. Commonly used drugs are chlorpheniramine (chlorpheniramine) 4mg/ time, 3 times /d, and children 0.35mg/(kg? d); Cyproheptadine 2mg/ time, 3 times/d; Diphenhydramine 25mg/ time, 3 times/d; You can also choose drugs without central sedative side effects, such as terfenadine 60mg/ time, twice a day; Cetirizine 10mg/ time, 1 time/day. The two drugs can be used alternately or in combination, and can also be used in combination with H2 receptor antagonists to enhance the curative effect.

② Nonspecific desensitization therapy: Acute or subacute eczema can be treated with 10% calcium gluconate 10ml or 10% sodium thiosulfate 10ml, 1 time /d; Or 0.25% procaine 20ml plus vitamin c 1 ~ 2g intravenously, 1 time/d.

③ Glucocorticoid: Generally speaking, it is not recommended. For acute inflammation and systemic inflammation, short-term use, such as prednisone 20 ~ 40 mg/d, can be considered, and after taking effect, it can be reduced or even stopped as appropriate.

④ Immunomodulators: freeze-dried BCG, thymosin, transfer factor, levamisole, etc. Optional, effective for chronic eczema, especially for elderly patients.

⑤ Antibiotics: Erythromycin, ciprofloxacin or intramuscular penicillin can be used for patients with bacterial infection, fever and lymphadenopathy.

3. Physical therapy and radiotherapy, liquid nitrogen cryotherapy, body surface X-ray or radionuclide (32P or 90Sr) application therapy, etc. It can be used for chronic localized eczema with long course of disease and stubbornness.

3. Local treatment

1) Acute phase:

① At the initial stage of the disease, there were only flushing, papules or a few small blisters, and there was no exudation. Therefore, it is advisable to diminish inflammation and avoid irritation. You can choose wet compress or lotion with antipruritic effect, such as wet compress with 2% ~ 3% boric acid water and calamine lotion or 2% camphor (borneol) and 5% alum calamine lotion. If it's just flushing, use boric acid talcum powder or dampness removing powder or talcum powder 30g, mirabilite powder 10g, and linalool (borneol) 2g to mix evenly and spray frequently, many times a day.

② For those with obvious blister erosion and exudation, it is advisable to converge and diminish inflammation to promote the recovery of epidermis. The antiseptic astringent liquid can be wet-packed or evaporated in bags, such as compound zinc-copper solution, 2% ~ 3% boric acid solution, 0.5% lead acetate or aluminum acetate solution, and purslane soup (60g with 2000~3000ml of water, boiled 15 ~ 20min). Decoct Cortex Phellodendri and Radix Sanguisorbae in water (30g for Cortex Phellodendri and 30g for Radix Sanguisorbae, decocted with 1000ml water 15 ~ 20min, and cooled for later use). Or decocted with Taraxacum mongolicum, Radix Gentianae and Flos Chrysanthemi Indici. (same as above). Secondary infection can be treated with 0. 1% ethacridine (rivanol) solution. Zinc oxide oil, 10% bismuth zinc oxide gallate ointment, Qushi powder or Sanmiao powder can be mixed with vegetable oil or licorice oil for external use.

③ The desquamation period at the end of acute eczema, if not handled properly, can easily delay the recurrence of the disease or turn into a chronic disease. The principle of treatment in this period should be to protect skin lesions, avoid external stimulation, promote keratin regeneration and eliminate residual inflammation. Cold ointment, coptis ointment or general emulsion can be used.

2) Subacute stage: The treatment principle is anti-inflammatory, antipruritic, dryness and dampness convergence, and zinc oxide oil, mud paste or emulsion are all applicable. According to the severity of exudation and erosion, whether the skin has infiltration, hypertrophy and itching, appropriate astringents, cutin promoters and antipruritics can be added, such as 3% black bean distillate, 5% Arnebia euchroma, 10% Sanguisorba zinc oxide oil, 2% linalool (borneol) and 5% black bean distillate, or can be used externally.

3) Chronic eczema: The treatment principle is to relieve itching, inhibit the proliferation of epidermal cells and promote the infiltration and absorption of dermal inflammation. Ointment, emulsion and mud paste should be selected. According to the degree of itching and skin hypertrophy, antipruritic agents, cutin accelerators or cutin dissolving agents with different concentrations are added, such as 5% ~ 10% compound pine oil ointment, 2% camphor (borneol), 10% ~ 20% black bean distillate ointment corticosteroid emulsion, etc. Or mix coptis paste and triamcinolone acetonide urea cream for external application.

prognosis

Eczema recurs and the course of disease is prolonged. The reason is difficult to trace back and eliminate. Acute eczema can be cured in about 2 ~ 3 weeks after proper treatment. In the subacute stage of chronic eczema, it is cured after several weeks, but it usually recurs.

Eczema prevention

(1) Although the cause of eczema is not clear, it is necessary to inquire about the medical history in detail, make necessary systematic examination, and try our best to find out the possible cause and remove it.

(2) People with allergic constitution should not only avoid contact with substances that are easy to cause allergies in food, clothing, housing and transportation (see allergens in the cause for details), but also strengthen physical exercise in order to improve their allergic state.

2, avoid stimulating factors Once eczema occurs, as a patient, we should try to avoid stimulating factors, including scratching, boiling water burns, soap scrubbing, drinking and spicy food, so as not to aggravate the condition of eczema.

3, early diagnosis, early treatment According to the characteristics of severe itching, multiform injury, exudation tendency, prone to limb flexion and recurrence, it is easy to diagnose. The densely distributed red papules and papules with severe itching should be treated as soon as possible. Because early eczema mostly belongs to acute phase or subacute phase, external use should follow the principle of corresponding order.

4, allergic constitution or allergic family history, to avoid all kinds of external stimuli, such as hot water burns, scratching, sun exposure and so on. Try to avoid allergies and irritating foods.

5, life should be regular, pay attention to work and rest.

6, clothing should be loose, in order to reduce friction stimulation, do not let chemical fiber and wool products directly contact the skin.

7. Eczema, especially chronic patients, has not recovered after years of treatment, and patients often lose confidence. In fact, eczema is not an incurable disease, because the cause of this disease is extremely complicated, which brings difficulties to treatment. Patients can apply Fuyanqing to maintain emotional stability and establish confidence in healing.
