In cardiovascular diseases, the poor increase of R wave refers to the abnormal ECG manifestations observed in electrocardiogram, especially in the precordial leads, which are usually related to amyloidosis involving coronary arteries. About 8% patients will have pathological Q wave or this R wave increasing phenomenon, which may cause angina symptoms. Amyloid cardiomyopathy is a kind of myocardial disease caused by the deposition of amyloid in heart tissue. Its pathogenesis is not clear, but it is related to extracellular protein folding errors, immunity, heredity and other factors.

In clinical examination, the bad signal of increasing R wave in the transition zone may indicate the existence of early amyloidosis, which is characterized by persistent aggravation of heart failure, left ventricular hypertrophy, low voltage of electrocardiogram, decreased wall activity and so on. In addition, specific medical history features such as hypertension, progressive hypotension and myocardial infarction patterns may also be diagnostic clues. The symptoms of angina pectoris are varied. Typical manifestations include severe chest pain, which may radiate to the left shoulder and upper limbs. Atypical symptoms may be more difficult to identify, which may be manifested as pain in the lower sternum or upper abdomen, or just discomfort and tightness.

Endocardial biopsy and histochemical staining are usually needed to diagnose amyloidosis. Therefore, it is very important to observe the adverse increase of R wave, combined with medical history and further examination.
