Therefore, although cough, expectoration, wheezing and wheezing are the main lung symptoms, their essence lies in kidney deficiency. According to Neijing, the qi of chronic bronchitis will be deficient when pathogenic factors are combined. Through research, Mr. Wang found that kidney deficiency objectively manifested as a significant decline in sex hormone level, cellular immune function and pulmonary ventilation function, and modern research also confirmed that asthma patients had "hidden kidney deficiency syndrome" (that is, hypothalamus-pituitary-adrenal cortex function was low) regardless of whether there was kidney deficiency witness. Mr. Wang thinks that phlegm-drinking disease is mostly "yang deficiency and yin excess" in the early stage, but due to long-term cough, a large amount of phlegm is discharged and body fluid is consumed for a long time; Or because of phlegm and blood stasis and internal heat, body fluid is consumed; Or body fluid loss due to repeated feeling of wind-heat or dryness-heat; Or because of frequent use of hormones or antiasthmatic drugs, it produces the image of yin deficiency and fire flourishing; Therefore, phlegm-drinking disease lasts for a long time, which may be deficiency of both qi and yin, or deficiency of both yin and yang, and there is internal resistance to phlegm-drinking. Mr. Wang calls this kind of symptoms "yin deficiency and phlegm retention syndrome". The common symptoms are: chronic cough with asthma, cough with more or less phlegm, poor or easy cough, asthma, loss of appetite, dry mouth and dislike of hot drinks, rotten stool, or diarrhea with cold and heat, spontaneous sweating and night sweats; The moss is thin and thin, with little moss or fluid, light or dark red tongue, cracks and thin and thin pulse. Diagnosis and treatment of cough.
Cough is one of the main symptoms of lung diseases. Cough syndrome differentiation first points to exogenous internal injury. Exogenous cough, which originates from violence, has six-qi syndrome; Cough due to internal injuries is mostly accumulated gradually, and every time there is a syndrome of five evils combined with five internal organs; Cough syndrome differentiation, cough sound is clear and turbid, phlegm color is qualitative, and it is divided into deficiency and excess, cold and heat; However, people who cough for a long time are often mixed with cold and heat, so they should be diagnosed by four methods and judged correctly. The key to treating cough: to treat exogenous cough, the medicine can't be static, and static will keep evil; Cough due to internal injury, inappropriate medication, and weak fire rising. Avoid warm and dry, robbing yin, bitter and cold, nourishing greasy products. Second, it is important to protect the spleen and stomach in treating phlegm.
Mr. Chang Yun: "Those who treat phlegm and drink but not spleen and stomach are not treated by it." The production of phlegm drink is related to the imbalance of lung, spleen and kidney function. Repeated cough and asthma, lung, spleen and kidney deficiency. Regular use of various drugs will damage the spleen and stomach, often leading to anorexia, abdominal distension, belching, loose stool and other symptoms. Therefore, before treatment, patients should be carefully asked about their diet and defecation, so as to check the status of spleen and stomach. The spleen and stomach are mainly regulated by four gentlemen, and those with excessive phlegm are added with Magnolia Officinalis, Pericarpium Citri Tangerinae, Rhizoma Pinelliae and Rhizoma Atractylodis. Third, it is helpful to treat phlegm, regulate qi and promote blood circulation.
Cough recurs, lung qi is blocked, phlegm and qi are mixed, and qi stagnation and blood stasis cannot be cured for a long time. In clinical practice, common patients have dark tongue, or ecchymosis, black lips, chest tightness and pain. Later, due to excessive phlegm, weak heart yang and impassable heart pulse, cyanosis appeared on the face, lips, tongue and nails, and even dyspnea, sweating and disharmony between yin and yang. Therefore, when the methods of treating cough, relieving asthma, resolving phlegm, relieving asthma and strengthening body resistance are applied, the products of regulating qi and blood, promoting blood circulation and removing blood stasis can be added. This is to treat phlegm first, qi can be eliminated by itself, and qi can also promote blood circulation. Early application is helpful to the divergence of lung qi, and the qi can rise and fall normally, which can prevent the disease from prolonging; After later application, qi and blood circulation and viscera function can be maintained normally. In recent years, domestic research shows that the method of promoting blood circulation and removing blood stasis can improve microcirculation, increase blood flow, improve blood supply of lung tissue, correct abnormal hemorheology and prevent emphysema and cor pulmonale. Medication characteristics

Mr. Wang believes that due to its cold nature and the special fishy smell of decoction and dry powder, the conventional dose (6 ~ 9 g) is not effective in relieving cough and asthma. If the dosage is increased, it will easily lead to gastrointestinal side effects, especially for children and elderly patients with poor spleen and stomach function, which is even more unacceptable. Therefore, Mr. Wang constantly reformed the dosage form of earthworm, and successively treated chronic bronchitis and asthma with injection, compound Longxing tablets and Tommy Tam oral liquid, and achieved good results. Second, reuse paint, eliminate phlegm and eliminate drinking.
Zeqi was first seen in Shennong's Classic of Materia Medica, "The main skin is hot, the ascites is heavy, the limbs are swollen, and the husband's yin is insufficient." Compendium of Materia Medica records that "lacquer tastes bitter, slightly cold and nontoxic". "Synopsis of the Golden Chamber" used Zeqi Decoction to treat cough caused by excessive phlegm and heavy pulse. In many years of clinical practice, Mr. Wang found that it is a cough-relieving and expectorant drug with definite curative effect. Crystal ⅰ and crystal ⅱ of Euphorbia helioscopia extract are monomers for relieving cough and eliminating phlegm in Euphorbia helioscopia. Zeqi extract and Ze -6 powder can reduce acidic mucopolysaccharide fibers, exfoliated epithelial cells and necrotic epithelial cells in sputum, suggesting its expectorant effect. Crystal D has obvious expectorant effect, and powder D has good expectorant effect in pharmacology, but it has great diarrhea side effects. In the observation, it was found that other extracts except D powder were prone to dry mouth reaction after taking, which was mild, especially D crystal, and had the effect of warming and resolving phlegm. Diarrhea occurred in the whole group of powder D, but there was no dry mouth reaction, which indicated that it was cold and had the effect of heavy diarrhea and expelling drinking. The usual dosage of Euphorbia helioscopia is 30 ~ 60g.
After long-term observation, the decoction and extract of Euphorbia helioscopia have no purgative and diuretic effects, and no toxic reaction was found in the function tests of heart, liver, kidney and hematopoietic system before and after treatment. The main side effects of Zeqi are stomach discomfort, dry mouth and dizziness, with an incidence of 6.08%. Therefore, people with stomachache, old age and weak spleen and stomach should use it with caution or in small doses. There have been reports of upper gastrointestinal bleeding caused by improper medication.
Three. Ephedra and ephedra root-declaration and merger.
Ephedra is an important drug for treating lung diseases, which has the function of relieving exterior syndrome and relieving asthma. The following questions should be paid attention to when using: 1) dosage: Ephedra should be used, tailored to different people. Attention should be paid to the dosage of urban people, the physically weak, the elderly and children. Especially in the treatment of chronic bronchitis, because the patients are older, they are often accompanied by cardiovascular diseases, hypertension, or have a history of prostatic hypertrophy, and the dose should not be large, generally 3-6g. Taking a large amount of ephedra is easy to consume lung qi, and there are symptoms such as increased heart rate, elevated blood pressure, insomnia, unfavorable urination, frequent urination and so on. Therefore, before use, ask the medical history; For long-term users, the dose should not be greater than 5g. 2) drug selection; Sweating, dispelling cold and relieving exterior syndrome can be used with raw ephedra, which has strong pungent dispersing effect; Dispelling lung and relieving asthma, and mixing with roasted Herba Ephedrae. 3) Ephedra combined with Radix Paeoniae Alba or Radix Glycyrrhizae can reduce its side effects. Ephedra root, the ancients thought it was dedicated to antiperspirant. In 1970s, it was reported that ephedra could relieve cough, phlegm and asthma. According to Mr. Wang's experience, the use of ephedra and ephedra root can not only relieve asthma and spread lung, but also supervise each other, such as ephedra sweating, ephedra root sweating, ephedra increasing blood pressure and ephedra root lowering blood pressure. For patients with cough and asthma accompanied by excessive sweating, these two drugs are often used together. Those who can't tolerate ephedra can be replaced by ephedra root.
4. Cicada and Bombyx Batryticatus-Qufeng Allergy
Cicada, sweet and cold, enters lung and liver meridians. It has the effects of dispelling pathogenic wind, clearing away heat, relieving sore throat, relieving convulsion and stopping epilepsy. Pharmacological experiments show that it has sedative effect, can reduce the tension of striated muscle, can also reduce reflex reaction, and has the blocking effect of ganglion. Bombyx batryticatus, salty, pungent and flat, enters the lungs and lung meridians. He is good at calming wind and relieving spasm, dispelling wind and clearing heat, resolving phlegm and resolving stagnation. Pharmacological analysis: protein contained in Bombyx Batryticatus can stimulate adrenal cortex and kill bacteria. Mr. Wang often uses two drugs to treat lung diseases. He believes that the two drugs can not only clear lung qi, dispel wind and dissipate heat, but also calm and relieve spasm, and also have anti-allergic and anti-virus effects. Dispelling wind, resolving phlegm and relieving sore throat can be combined with Fructus Arctii, Radix Platycodi and Radix Glycyrrhizae; Clearing heat and relieving cough can be combined with Fructus Forsythiae, Bupleuri Radix and Scutellariae Radix; Cough for a long time is accompanied by itchy throat because of wind evil and love for the lungs. For Su Xuan, it can be combined with Tianzhu Zi and Chimonanthus praecox. Clearing heat and relieving asthma can be compatible with Belamcanda and Ephedra; It can dispel wind, cool blood, promote diuresis and relieve itching. It can be used with Cortex Moutan, Cortex Dictamni Radicis and Kochia scoparia. It can clear away heat, improve eyesight and relieve headache. It can be used with mulberry leaves and Vitex negundo.

Scorpion, an important medicine for treating wind, has the functions of calming wind, relieving spasm, expelling wind and relieving pain, detoxicating and resolving hard mass; Scolopendra is good at searching wind, dredging collaterals, relieving spasm, detoxifying and relieving pain. Clinically, it is often used to treat acute and chronic convulsions, tetanus, rheumatalgia, headache and sores. Mr. Wang often uses two drugs to treat acute asthma attacks. He believes that the characteristics of asthma attack are sudden, paroxysmal and repetitive, which is similar to the characteristics of wind "good behavior and rapid change", so adding two drugs in treatment can often get good results. "Clinical Experimental Study on Kidney Deficiency and Essence of Chronic Bronchitis and Drugs for Nourishing Kidney and Supplementing Qi" won the third prize of scientific and technological achievements of Shanghai University of Traditional Chinese Medicine.
(1) In this study, the internal relationship between the essence of kidney deficiency in chronic bronchitis and pulmonary ventilation function, gonadal function, cellular immune function and some plasma proteases was discussed.
