1. The newly-opened catering industry must submit the following materials to apply for the Hygiene Permit:
1. Application for Hygiene Permit;
2, the construction project design hygiene review approval;
3, the completion of the construction project health acceptance letter;
4. The plan layout of the business premises of the catering business unit (indicating the positions of kitchen function rooms, warehouses and restaurants and their corresponding areas);
5. List of sanitary facilities;
6, water quality hygiene inspection report;
7. Certificate of health examination and health training for employees;
8. Health management organization and system;
9. Other materials required by laws, regulations, rules and standards.
2. The hygiene license of catering enterprises is valid for two years, and it is reviewed once a year. The review time is based on the issuance time of the hygiene license, and it is declared 31 days in advance each time, and the following materials are submitted:
1. Application for hygiene license;
2. A letter of commitment that the name, legal representative, business items, layout and facilities of the catering industry are consistent with the original approval;
3. Qualification certificate of health examination and health training for employees in the new year;
4, water quality hygiene inspection report;
inspection report on tableware and food hygiene within five or six months;
6. Original (original and copy) of the original health permit;
7. Other materials required by laws, regulations, rules and standards.
3. If the catering business unit needs to renew the hygiene license (renewal), it should apply 31 days before the expiry of the hygiene license, and submit the application materials according to the following requirements:
1. Application for hygiene license;
2. A letter of commitment that the name, legal representative, business items, layout and facilities of the catering industry are consistent with the original approval;
3. Certificate of health examination and health training for employees;
4, water quality hygiene inspection report;
inspection report on tableware and food hygiene within five or six months;
6. Copy of the original hygiene license (original and duplicate);
7. Other materials required by laws, regulations, rules and standards.
Procedures for Handling Hygiene Permit for Catering Industry
1. All application materials are in duplicate, arranged in sequence, printed on A4 paper, and stamped with the official seal of the applicant one by one.
2. If there are errors in the application materials that can be corrected on the spot, the applicant is allowed to correct them on the spot.
3. If the application materials are incomplete or do not conform to the statutory form, the applicant shall be informed of all the contents that need to be supplemented at one time on the spot or within five working days. If the applicant meets the requirements after supplementing the materials, it can be accepted. Fails to inform, since the date of receipt of the materials shall be deemed to be accepted.
4. The administrative department of health shall make a decision on approval or disapproval within 21 working days from the date of acceptance. If a decision cannot be made within 21 working days, it may be extended for 11 working days upon approval, but the applicant shall be informed of the reasons for the extension. If there are other provisions in laws and regulations, the provisions shall be followed, and inspection, inspection, quarantine, appraisal and expert review are required according to law, the time required shall not be counted in the period of health administrative license.
