3. Copy of the ID cards of the person in charge and the food safety management personnel (it is best that the person in charge and the food safety management personnel are the operators themselves, and large enterprises can set them separately)
4. List of business equipment and tools suitable for food business
5. Spatial layout of business facilities suitable for food business
6. Commitment letter of food operators and food safety management system

The accepting personnel are responsible for receiving the license application put forward by the applicant, reviewing whether the application materials are complete and conform to the legal form, and signing the acceptance opinions. To be accepted, sign in the column of "review opinions" of the application submitted by the applicant "After review, the materials submitted by the applicant are complete and conform to the statutory form, and the application conditions meet the requirements of Items 1 to 4 of Article 27 of the Food Safety Law and the requirements stipulated by law. It is recommended to grant permission. " If it is not accepted, it shall refer to the relevant terms to be accepted and sign an opinion in reverse, explain the reasons and inform the right to appeal.

after acceptance, the market supervision administration of each district bureau will assign personnel to conduct information and on-site review according to the Food Safety Law of the People's Republic of China, the Implementation Regulations of the Food Safety Law of the People's Republic of China and other requirements. The auditor shall review the licensing matters submitted for application, and after passing the examination, appoint on-site inspection organs or personnel according to the principle of jurisdiction, and the on-site inspectors shall conduct on-site inspection, fill in the on-site inspection registration form and sign the inspection opinions. Acceptor and reviewer shall not be the same person.
IV. Approve
The market supervision and administration bureau of each district bureau makes the approval decision of approving (or not approving) the administrative license according to the audit results. The approver shall review all the license materials submitted for approval and sign the approval opinions. If it is decided to grant the license, it shall be signed in the column of approval opinions of the application submitted by the applicant. After examination, the materials submitted by the applicant are complete and conform to the statutory form, and the application conditions meet the requirements of Items 1 to 4 of Article 27 of the Food Safety Law and the requirements stipulated by law. Agree to grant permission. If it is decided not to grant the license, it shall refer to the relevant terms of granting and sign the opinions in reverse, explain the reasons and inform the right of action. The approver and approver can be the same person.
v. Certification
If a decision of approval is made and a Food Circulation Permit should be issued, a Food Circulation Permit will be issued to the business operators. When issuing certificates, the issuing personnel shall review the corresponding notice and identity certificate of the recipient, and issue and sign it after verification. It is stipulated that the license information entered into the food circulation license management system shall be true, complete, accurate and timely.
To apply for a food circulation license, the following materials shall be submitted:
1. An Application for a Food Circulation License;
2. A copy of the Notice of Name Pre-approval;
3. Relevant certificates of legal use of the business premises;
4. the identity certificate of the person in charge;
5. Power of attorney and identification of the entrusted agent or designated representative;
6, food safety management system;
7. List of food business equipment suitable for its business activities;
8. provide copies of health certificates and health regulations training certificates for food employees;
9. Operators applying for dairy products projects shall also submit the certificate of the bank opening the account and a written statement;
11, full-time or part-time food safety management personnel's identity certificate (junior high school education certificate and training certificate);
11. Other materials required by the industry and commerce department.

Measures for the Administration of Food Business License
Article 2 Anyone who engages in food sales and catering services within the territory of the People's Republic of China shall obtain a food business license according to law.
these measures shall apply to the application, acceptance, examination, decision, supervision and inspection of food business license.
Article 12 To apply for a food business license, the following materials shall be submitted to the local food and drug supervision and administration department at or above the county level where the applicant is located:
(1) An application for a food business license;
(2) a copy of the business license or other qualification certification documents;
(3) documents such as the layout of major equipment and facilities, operation procedures and so on, which are suitable for food business;
(4) rules and regulations to ensure food safety, such as food safety self-inspection, health management of employees, incoming inspection records, and handling of food safety accidents.
if the vending equipment is used to sell food, the applicant shall also submit the product qualification certificate of the vending equipment, the specific location, the name, address, contact information of the operator, the publicity method of the food business license and other materials.
if the applicant entrusts others to apply for a food business license, the agent shall submit the power of attorney and the identity certificate of the agent.
