Method:
1. Identity certificate: provide the original and photocopy of household registration book and ID card (the photocopy of household registration book includes the first page of household name and the insured page), the original and photocopy of Hong Kong and Macao students' travel permit for Hong Kong and Macao residents to and from the mainland, the original and photocopy of mainland travel permit for taiwan residents for Taiwan Province students and the original and photocopy of passport for foreign students.
2, fill in the "urban residents basic medical insurance payment declaration form" in duplicate (to the insurance registration department to receive or visit the local social insurance fund management center website to download)
3. If it is necessary to apply for bank transfer payment, provide the original and photocopy of the savings card or current passbook of any resident medical insurance payment bank, and the original and photocopy of the ID card of the head of household corresponding to the savings card or passbook, and fill in and sign the Authorization Letter for Entrusting the Bank to Pay Social Insurance Fees by Automatic Transfer (get it from the affiliated insurance registration department) according to the prescribed format.
4. College students: Among them, students with difficulties need to provide the Minimum Living Guarantee Certificate for Urban and Rural Residents or the Certificate of Low-income Families with Difficulties issued by the civil affairs department where the students are registered, or the original and three copies of the Disabled Persons' Card of People's Republic of China (PRC) issued by the Disabled Persons' Federation. /5. Primary and secondary school students: Among them, those who have reached the age of 18 before June 30 of that year and are enrolled as primary and secondary school students for the first time, as well as those who are not registered in this city, are required to provide the school registration certificate issued by the school.
6. Among them, the newborn who has registered for insurance within three months after birth should also provide the original and photocopy of the birth medical certificate. If the first three months after the birth of a newborn spans two social security years and fails to go through the medical insurance procedures for residents in the year of birth in time, the legal guardian of the newborn can submit an application report for paying the medical insurance fee for residents in the year of birth within three months after the birth of the newborn. Within 3 months after birth (including 3 months), you can enjoy the corresponding medical insurance benefits from the date of birth.
