Piriformis muscle injury syndrome
Piriformis syndrome refers to abnormal piriformis muscle stimulating or compressing the sciatic nerve, causing buttock and leg pain. It is often caused by ectopic sciatic nerve, passing through the piriformis muscle, and chronic injury of the piriformis muscle. The pain is mainly in the buttocks. Although there is sciatica in the lower limbs, the tenderness points are at the upper edge, lower edge, or between the upper and lower edges of the surface projection of the piriformis muscle on the affected side, and the cord-like tense piriformis muscle can be touched. The straight leg raise test is positive within 60 degrees, but negative after exceeding 60 degrees. Internal rotation and external rotation of the affected lower limb, as well as movements that stretch the sciatic nerve, can aggravate the pain and cause radiating pain.
Treatment: External application of Huoxue Powder and Jiegu Powder. Or apply plasters that activate blood circulation, unblock collaterals, reduce inflammation and relieve pain, or take Chinese herbal medicine that activates blood circulation and removes blood stasis. Massage, acupuncture or
partial sealing.
When ineffective, surgery may be considered to relieve the compression of the sciatic nerve by the piriformis muscle.
