Learn to use the ideas and methods of classics.
The best way to learn classics is to study and explore basic classics. The basic prescription refers to the general prescription mainly aimed at the pathological properties of viscera. The first way to learn the usage of moving meridian is to learn to use the basic formula of moving meridian, which is not only fixed, stable and concrete, but also regular, universal and feasible. Only by paying attention to the in-depth study of basic prescriptions can we lay a solid ideological foundation for the application of classic prescriptions; Only by learning and understanding the foundation can we master and use it. The biggest advantage of selecting the basic prescription for syndrome differentiation and treatment is that it is aimed at the nature of the disease rather than confined to the lesion, especially in the case of extensive and uncertain lesion.
I summarized the common basic prescriptions. It can be said that in clinic, if Sinisan is the basic prescription for regulating qi and relieving depression, then all patients with qi depression can first consider using Sinisan.
Guizhi Fuling pill, as the basic prescription of promoting blood circulation and removing blood stasis, in other words, Guizhi Fuling pill can be used as the first choice for treating all blood stasis in the process of disease.
There are four kinds of medicines in Akamaru, namely, aconite, pinellia tuber, poria cocos and asarum, which can warm the cold and resolve phlegm. There are two kinds of phlegm in clinic, one is cold phlegm and the other is hot phlegm. Xiao Xiong Hot Phlegm Decoction is the basic prescription for clearing away heat and resolving phlegm, and cold phlegm is Akamaru.
If a person has damp-heat, choose a prescription to treat damp-heat, which is called Zhi Zi White Skin Decoction. Zhi Zi Baipi Decoction comprises Fructus Gardeniae, Cortex Phellodendri and Radix Glycyrrhizae. In clinical practice, as long as a person is damp-heat, Zhi Zi Baipi Decoction is the first choice. A patient came to see a doctor, with damp heat in the lower energizer and blood stasis. We chose two basic prescriptions, Zhizi Baipi Decoction and Guizhi Fuling Pill.
The basic prescription for treating cold and dampness is Ganjiang Lingzhu Decoction, which contains four kinds of medicines: licorice, dried ginger, poria cocos and Atractylodes macrocephala. For example, when I was working in the clinic, I met a lesbian. She said that the amount of colorful white should come from the cold and dampness of our Chinese medicine. Then she said that her illness had a lot to do with her mood. When she was unhappy, pelvic inflammatory disease recurred and she was in a good mood, but she was not well, but she was mild. In this case, there are two prescriptions, one is Ganjianglingshu decoction, and the other is Ganjianglingshu decoction. In the outpatient clinic, I also met such a patient with chronic pelvic inflammatory disease and increased leucorrhea. He often felt that his lower abdomen was as painful as acupuncture, so he prescribed Ganjianglingshu Decoction and Guizhi Fuling Pill, which is the importance of learning the basic prescription.
Zhang Zhongjing has a prescription called Shaoyao Gancao Decoction. Peony nourishes blood and licorice nourishes qi. The so-called basic prescription means that there are many clinical diseases. As long as it appears, blood deficiency and qi deficiency can be added.
The basic prescription of nourishing blood and warming yang is Danggui Sini Decoction. The basic prescription of warming yang and strengthening yang is Sini decoction. Recently, I met a patient whose tongue was cold on the right side for the first half of the day, cold on the left side for the second half, and cold around at night. She said it was colder than eating ice cream, and the ice made her teeth uncomfortable, and her upper jaw uncomfortable, especially uncomfortable. There are no problems in all kinds of tests. Western medicine says it's menopause and you can't take medicine. At that time, I saw that her tongue was pale and white, so I gave her Sini decoction. Sini decoction has three medicines: aconite, roasted licorice and dried ginger. The aconite used in Sini decoction is raw. I gave her five grams of raw aconite, five grams of dried ginger and six grams of roasted licorice, told her to boil it with strong fire, and then calculated the time, which should not exceed 10 minute. When she took the medicine, the pharmacy said it would take at least 1 hour, and I told her it would take about 10 minutes. She came to see me again. She said that you told me that the decoction would take about 10 minutes. The drugstore said it would take a long time to decoct the medicine. She asked me how long it would take. I said that this prescription is the prescription of the ancients, and the medicine used should be carefully decocted when it plays a therapeutic role. The patient took the medicine for a week. When she saw a doctor the next week, she said that her tongue was no longer cold. At that time, there were several graduate students in the outpatient department, so I asked her how she felt after drinking the medicine. She said that she felt nothing after taking the medicine. Because my tongue was cold, I didn't feel bitter or anything, so I stuffed it in my mouth. One of the graduate students asked her how she felt after taking the medicine. She said that she didn't feel anything, but she felt that her tongue was getting colder and colder, so that it was no longer cold, and she wanted to consolidate the treatment.
The basic prescription of nourishing yin and cooling blood is Lily Dihuang decoction.
The basic prescription of clearing heat and purging fire is Baihu decoction.
Dachengqi decoction is a basic prescription for purging heat and dredging fu-organs. I often prescribe Dachengqi decoction to treat acne. Dachengqi decoction has a good therapeutic effect, but it must be a heat knot here.
The basic prescription of Wenyang Xiatong is Dahuang Fuzi Decoction.
The basic prescription for eliminating dampness is Oyster Alisma Powder.
Zhenwu decoction is the basic prescription of warm water and qi.
To learn Zhang Zhongjing's methods well, Zhang Zhongjing's basic methods must be firmly remembered. Compared with Guizhi Fuling Pill, we regard it as the basic prescription for promoting blood circulation and removing blood stasis. As long as the pathological attribute belongs to blood stasis, Guizhi Fuling Pill can be selected. Zhang Zhongjing's exposition on Guizhi Fuling Pill is mainly to treat gynecological diseases, but our application today is not limited to gynecological diseases.
There is a lesbian who has a lot of fat to prevent tumors. Lipoma is bigger than walnut and smaller than mung bean. According to the symptoms, I gave her Guizhi Fuling pills. After taking the medicine, the patient feels that the big one is shrinking and the small one is disappearing.
This year, there is a graduate student. She said that there are at least 50 small sarcomas on her lover's body, which are as painless as wheat grains, but her lover psychologically feels that there should be no sarcoma. I prescribed Guizhi Fuling pills for him, and added two kinds of medicines, one is called Semen Vaccariae and the other is called Gleditsia sinensis. Vaccaria seed promotes blood circulation, and Gleditsia sinensis is soft and firm for about two weeks. About a third of her sarcoma is gone. She said she needed treatment for a while. We can't just study Guizhi Fuling Pill in gynecology, as long as it is used for removing blood stasis. Guizhi Fuling Pill is relatively neutral. It is hot when it is hot, because it is cold when it is cassia twig, and it is cold when it is cold. Because it is peony bark, it is neither hot nor cold.
For those with depressed qi, Sinisan can be used.
The other is to represent the party. The so-called representative prescription refers to the unique prescription for visceral diseases selected from many prescriptions according to syndrome differentiation. To learn and use the basic principles of moving classics well, we must study the representative side deeply, which should be intensive, typical and unique, selective, targeted and reliable. Only by in-depth study of the representative prescription can we provide the best idea for the application of the classic prescription, that is to say, only by in-depth study and excavation of the representative prescription can we grasp and use the breakthrough point of the treatment of the representative prescription. The greatest advantage of selecting representative prescription for syndrome differentiation and treatment is that it is aimed at both the lesion site and the lesion attribute, that is, the lesion site of syndrome differentiation and treatment is fixed and certain.
Xiaoqinglong decoction is the first choice representative of lung cold syndrome. If we meet a patient with lung cold syndrome and a serious cold, we can add Akamaru; If it is lung cold syndrome and emotional excitement, Sinisan can be added. This is how we learn the classics. On the one hand, we should pay attention to the basic side, on the other hand, we should pay attention to the representative side.
Basic prescription, aiming at the nature of the lesion rather than the lesion site; Represents the principle of the nature and location of the lesion.
The representative prescription of lung cold syndrome is Xiaoqinglong decoction.
The representative prescription of lung-heat syndrome is Zeqi decoction.
The representative prescription of deficiency of lung, qi and yin is Maimendong decoction.
The representative prescription of spleen and stomach qi stagnation syndrome is orange orange ginger soup.
Lizhong Pill is the representative prescription of spleen-stomach deficiency and cold syndrome. For example, when I was working in an outpatient clinic, I met a patient and asked him what was wrong. He said it was chronic gastritis. Then I said that I haven't eaten cold food for many years, and it should be deficiency cold. When we saw that the tongue was reddish, the tongue coating was yellow and thick, and it was hot, we prescribed Lizhong Pill and Xiao Xiong Chest Decoction, and finally achieved the expected therapeutic effect.
The representative prescription of spleen-stomach cold-heat inclusion syndrome is Banxia Xiexin Decoction.
The representative prescription of spleen and stomach deficiency of both qi and blood is Huangqi Jianzhong decoction.
The representative prescription of liver depression and blood stasis syndrome is Dahuangchong Pill.
The representative prescription of liver damp-heat syndrome is Yinchenhao decoction.
The representative prescription of liver heat generating wind syndrome is Yin Feng decoction.
Xiaochaihu decoction is a representative prescription of liver and gallbladder stagnation and heat syndrome.
The representative prescription of kidney deficiency is Tianxiong Powder.
The representative prescription of kidney yin deficiency and water vapor syndrome is Zhu Ling decoction.
The representative prescription of kidney yin deficiency syndrome is Shenqi pill.
The representative prescription of heart-qi stagnation syndrome is Fructus Aurantii Immaturus Bulbus Allii Macrostemon Guizhi Decoction.
The representative prescription of heart-yang deficiency syndrome is Guizhi plus Fuzi decoction.
The representative prescription of deficiency of both heart and yin and yang is Zhigancao decoction.
The representative prescription of heart, liver, yin and blood deficiency syndrome is Suanzaoren Decoction.
The representative prescription of Yang Qi deficiency of heart and kidney is Fuling Sini decoction.
The representative prescription of heart-spleen wind-phlegm syndrome is Hou Shi Heisan.
The representative prescription of stasis-heat syndrome is Taohe Chengqi Decoction.
The representative prescription of cold blood stasis syndrome is Wenjing decoction.
The representative prescription of yang deficiency bleeding syndrome is Huang Tu decoction.
The representative prescription of blood deficiency bleeding syndrome is Jiao Tangai.
The representative prescription of wind-cold-dampness arthralgia is Aconitum Decoction.
The representative prescription of arthralgia syndrome due to yang deficiency and heat stagnation is Guizhi Shaoyao Mu Zhi Decoction.
Danggui Shaoyao Powder is a representative prescription of syndrome of deficiency of both qi and blood and dampness.
Although there are many diseases in clinic, there are also many syndromes of diseases, which are basically treated with basic prescription plus representative prescription.
For example, for example, the representative of deficiency of both yin and yang of the heart is roasted licorice soup. Can Zhigancao Decoction be used with Zhenwu Decoction? Zhigancao decoction can cause edema due to deficiency of both yin and yang, and edema should be distinguished from two aspects. If you have a cold, you should use Zhenwu decoction. If it is damp and hot, use Poria cocos and Alisma orientalis powder.
A heart disease will have dry stool. Dry stool should distinguish between cold and heat, what to add when it is hot, and what to choose when it is cold. If yin deficiency is the main factor, what should I choose? If yang deficiency is the main factor, what should I choose? If this person is yin-deficient, we can choose Lily Dihuang Decoction, and any representative can combine with the basic party, so that we can draw inferences from one another. For example, Xiaoqinglong Decoction can treat lung cold syndrome. Although there are many prescriptions for treating lung cold syndrome, Xiaoqinglong decoction is the best choice. It can be said that except Xiaoqinglong decoction, everything else is irreplaceable.
This is the derivative. The so-called derivation is to learn from the constant changes of the ancient side. Derivative refers to the prescription produced by the evolution of disease and syndrome in the dynamic process. The best way to enrich and expand prescriptions is to study derivatives in depth. Derivatives are not only inheritance, relevance and variability, but also randomness, specificity and operability. Only by in-depth study of derivatives can we optimize the best thinking of users, that is, only by in-depth thinking and exploring diseases from derivatives.
The so-called derivation means that we use Guizhi decoction and add pueraria lobata on the basis of Guizhi decoction; Adding Astragalus is called Guizhi plus Astragalus Decoction; You can add aconite. Why do you want to add astragalus, because the patient is qi-deficient; Why do you want to add aconite, because the patient has yang deficiency; If the patient coughs and breathes, add Magnolia Officinalis and Almond.
Zhang Zhongjing introduced several derivatives in Treatise on Febrile Diseases. One is cinnamon twig soup. According to my statistics, there are more than 20 kinds of derivatives of Guizhi Decoction. In addition, there are Mahuang Decoction, Xiaoqinglong Decoction, Lily Mu Zhi Decoction, Gardenia Soybean Decoction and other derivatives. The derivatives of Xiaochaihu Decoction include Chaihu Guizhi Decoction, Chaihu Guizhi Ganjiang Decoction, Chaihu Jiamirabilite Decoction and Chaihu Jiagulong Oyster Decoction. Dried ginger, Qin Huang, Coptidis Rhizoma and ginseng soup can evolve into Banxia Xiexin Decoction, Ginger Xiexin Decoction, Glycyrrhiza Xiexin Decoction, Wu Mei Pill, Dachengqi Decoction derivatives and dried ginger.
