To apply for a food business license, materials shall be submitted to the local food and drug supervision and administration department at or above the county level where the applicant is located:
1. An application for a food business license;
2. A copy of the business license or other qualification certification documents;

4, food safety self-inspection, health management of employees, incoming inspection records, food safety accident handling and other rules and regulations to ensure food safety;
5. In case of using vending equipment to sell food, the applicant shall also submit the product qualification certificate of the vending equipment, the specific location, the name, address, contact information of the operator, the publicity method of the food business license and other materials.
Conditions for handling food hygiene license:
1. Site selection: It must meet the relevant requirements of the Food Hygiene Law of the People's Republic of China. The place and the surrounding environment are clean and tidy, and there is no obvious pollution source;
2. We should establish and improve the health management system and organization of all kinds of production workshops, and equip full-time or part-time health management personnel;
3. Employees must hold valid health certificates and health knowledge training certificates;
4. There must be an exhaust system in all kinds of production workshops. All workshops with air conditioning devices must have fresh air supply and reasonable ventilation. The fresh air inlet should be located outdoors, away from pollution sources. Each bathroom is equipped with an independent exhaust system;
5. Set up health files: supervision documents, inspection reports, medical examination files for employees, files for transferring patients with infectious diseases, records of self-examination and self-inspection, declaration and approval materials;
6. Set up a health quality inspection institution suitable for the production capacity, and be equipped with inspectors who have obtained the inspection qualification through professional training;
7. The site selection, design, process layout, production technology, production equipment, sanitary facilities and water consumption of food production and business premises meet the requirements of relevant national laws, regulations, rules and sanitary standards;
8. Equipment, pipes, tools, containers and transportation tools that come into contact with food, and packaging materials of products must meet the hygiene requirements;
9. The raw materials, formula, production technology, product labels and instructions of the products meet the hygiene requirements;
11. The product has passed the inspection by the inspection organization recognized by the health administrative department.

Article 2
Anyone who engages in food sales and catering services within the territory of the People's Republic of China shall obtain a food business license according to law. These Measures shall apply to the application, acceptance, examination, decision and supervision and inspection of food business license.

The food business license shall follow the principles of legality, openness, fairness, impartiality, convenience and high efficiency. Article 4 The principle of "one place, one license" shall be applied to the food business license, that is, a food business operator shall obtain a food business license when engaging in food business activities in a business place.
