first, the conditions for handling the "health pass card". Employees and individual industrial and commercial households who live in Class II and III areas of our city and need to return to work may apply for a "health pass card" if they have no symptoms such as fever, persistent dry cough and fatigue for more than 1.4 consecutive days and exclude the following situations.

being diagnosed as a confirmed case, a suspected case, a positive infected person, not cured or less than 1.4 days after discharge in COVID-19;

The close contacts of confirmed cases, suspected cases and positive infected persons in COVID-19 have not been released from medical observation;

less than 14 days after returning to this city from Hubei and other areas with high epidemic incidence;

People who have had contact with people in areas with high epidemic incidence such as Hubei, and have been isolated and observed for less than 14 days.

second, the handling process. You can voluntarily declare in the following two forms.

enterprises declare on their behalf. Encourage employers to declare the "health pass card" on behalf of their employees as needed, and fill in the "Declaration Form for Tangshan Returned Workers" by one person.
