2. Prevention and control effect standards:

Note: ① Mosquitoes: The average number of adult mosquitoes in the positive room is less than 11%.

② Flies: The average number of flies in the positive room is less than 3%, and the number of live third-instar larvae on the infested ground of fly maggots is less than 2%

③ Cockroaches: the infestation of drugs (1.3% permethrin solution) is less than 5%, and the average number of Periplaneta is less than 5 per room.

④ Rats: two pieces of talcum powder with a size of 21 * 21cm per room of 15m2, and the positive rate is less than 5% after one night.

2. The above-mentioned positive rooms refer to rooms with pests: if the above-mentioned standards are changed during the contract period, this article shall be changed in line with the national laws and regulations.

article 3. rights, obligations and responsibilities of party a:

1. party a shall inform party b of its relevant rules and regulations, and have the right to require party b to strictly abide by them and supervise the pest control of party b's staff.

2. Party A shall appoint a health worker to assist Party B in the work, especially in the coordination among various departments within Party A: pay the prevention and control expenses on time and respect Party B's prevention and control procedures.

3. Party A shall do a good job in daily hygiene, remove accumulated water in time, and control the evils of mosquitoes and flies, so as to reduce the density; Garbage should be packed in bags and covered in barrels, and it should be cleaned every day, and the "three prevention" measures should be improved, and the pipe caves in the communicating parts inside and outside the building should be screened and grated to prevent rats from invading. The combination of prevention and treatment should be done well, and Party B's staff should make suggestions on pest prevention according to the site environment.

4. party a provides convenience for party b's staff to work at party a's site, such as water, electricity and other necessary conditions.

5. Party A shall promptly dispose of the bodies of mosquitoes, flies and cockroaches after each disinfection treatment by Party B..

Article 4. Rights, obligations and responsibilities of Party B:

1. Prevention and control procedures of Party B: according to the field situation, select disinfection equipment, drug dosage form, concentration and application method; Protect the safety of personnel, food, electrical equipment and other items; When disinfection is implemented, comprehensive measures should be taken to control the density of mosquitoes, flies, cockroaches and rats, such as indoor first, outdoor first, ground first, walls first, and key first.

2. Party B guarantees that its disinfection personnel will abide by Party A's rules and regulations, and that they can accept Party A's reasonable suggestions.

3. Party B shall use chemicals approved by relevant state departments, and shall not use counterfeit or "three-no" products, and shall accept the supervision of Party A at any time.

4. As pests and rats are a dynamic process, if Party A finds any problems with mosquitoes, flies, cockroaches and rats after Party B's regular disinfection, it can contact Party B in time, and Party B will send staff to deal with them in time.

Article 5: Disinfection frequency, charge and payment method:

1. Disinfection frequency: Party B promises to carry out disinfection regularly within the time agreed by both parties, 1 times every October and 1 weeks; The pest control depends on the rodent damage.

2. Charge: Party B provides Party A with the service items in the contract, and the service charge is ￥ 3,611.11 per year; (Say three thousand six hundred yuan only).

3. payment method; Party A shall pay Party B the prevention and control fee for the last quarter on a quarterly basis. Unless there is another objection to the accounts, it shall pay it within seven working days after receiving the invoice from Party B. For the overdue payment of Party A, it shall pay Party B a penalty of one thousandth of the arrears every day.

article 6. termination and extension of the contract:

1. the party that terminates the contract without authorization shall pay 31% of the total amount of prevention and control service fees as liquidated damages to the other party during the contract period. however, due to the overdue payment of party a for more than one month, party b has the right to unilaterally terminate the contract and demand party a to pay the arrears and liquidated damages according to the relevant provisions of this contract.

2. The modification or termination of the contract shall be agreed one month before the expiration of the contract, and the proposing party must notify the other party in writing one month in advance, otherwise it will be regarded as an automatic extension of the contract, and each automatic extension period is one year.

Article 7. Liability accident:

1. If Party A fails to take measures to ensure that it meets the requirements specified in Article 3, or this contract cannot be performed due to Party A's reasons, so that Party A fails to meet the standards in the inspection and acceptance conducted by relevant departments at all levels, Party A will take full responsibility; However, if the prevention and control effect cannot be recognized by the relevant departments due to Party B's poor service, Party A has the right to refuse to pay the prevention and control fee, and has the right to demand Party B to compensate the corresponding losses (which means being fined by the relevant departments).

2. Party B shall be responsible for the injuries, accidents or other similar accidents caused by Party B's staff's violation of operating rules or dereliction of duty when performing disinfection services at Party A's site; For accidents caused by the fault of Party A or Party A's staff, Party A shall bear the corresponding responsibilities.

3. losses caused by force majeure shall be handled in accordance with national laws and regulations.

article 8: this contract is made in duplicate, each party holds one copy, which has the same legal effect. any party's proposal to modify the contract must be agreed and recorded by both parties, otherwise it will be deemed invalid.
