Legal basis: Article 25 of the Detailed Rules for the Implementation of the Regulations on the Administration of Medical Institutions. To apply for the practice registration of medical institutions, you must fill in the Application for Practice Registration of Medical Institutions and submit the following materials to the registration authority:
(a) the approval of the establishment of a medical institution or the receipt of the establishment of a medical institution;
(two) the certificate of ownership or use of the house used by the medical institution;
(3) Architectural design scheme of medical institutions;
(4) capital verification certificate and asset evaluation report;
(5) Rules and regulations of medical institutions;
(6) A list of the legal representative or principal responsible person of the medical institution and the responsible persons of various departments, and copies of relevant qualification certificates and practice certificates;
(seven) other materials provided by the administrative departments of health and family planning of provinces, autonomous regions and municipalities directly under the central government.
To apply for the registration of outpatient department, clinic, health center, infirmary, health center and health station, it shall also submit the list of drugs attached to pharmacy (cabinet), the list of health technicians and their relevant qualification certificates, photocopies of practice certificates and other materials required by the health and family planning administrative departments of provinces, autonomous regions and municipalities directly under the Central Government.
