The method of drawing the questionnaire for primary school students (for example) is as follows:

1. Tools/materials:

Computer and operating system: Windows7.

Software: word2013, Excel2013.

2. Method/steps

Open word2013 and enter the title of the survey on the first line: Primary School Students Reading Questionnaire - then enter the purpose of the survey at the beginning of the text - the title font is 28 , the text font is 20

Click Insert - Table - Insert table - Number of columns is 2, number of rows is 10 - Set fixed width - Click OK.

Set the table font to 20 at the beginning - enter column titles, survey questions, and question options.

Then enter the question and the corresponding options - click in front of option A - click Insert - Symbols - Other symbols.

Click on the symbol - subset selection: Geometric symbol - select the hollow square - click on Insert.

Then select the hollow square you just inserted - click Copy on the Start menu - and paste it in front of other options. Other questions and options are designed similarly.

Enter the survey title and purpose of the survey.

Insert the table with 2 columns and 10 rows. ?

Set the table font and enter column titles.

Click on option A to insert other symbols.

Select the hollow square.

Copy the hollow square to other options.
